    • A cardiac implant (20, 120, 220, 320, 520, 620, 720, 820, 920) is provided for treating diastolic heart failure, the cardiac implant including a flexible tether (24, 124, 224A, 224B, 324); a superior anchor (26, 226A, 226B), which is coupled to a superior portion (28, 128) of the flexible tether and configured to anchor the flexible tether to a left- atrial site (46) of a wall (30) of a left atrium (32) of a heart (14); and an inferior anchor (34, 234A, 234B), which is coupled to an inferior portion (36, 236A, 236B) of the flexible tether and configured to anchor the flexible tether to a left-ventricular site (38) of a left ventricle (56) of the heart (14). The flexible tether (24, 124, 224A, 224B, 324) is configured such that when the flexible tether is anchored and has a length of 6 - 12 cm between the superior anchor (26, 226A, 226B) and the inferior anchor (34, 234A, 234B), the flexible tether reduces a volume of the left atrium (32) during at least a portion of ventricular diastole of each cardiac cycle, thereby enhancing ventricular filling.     • Apparatus (20) is provided comprising a skin-application portion (40), configured to move across skin of a subject. At least one acoustic element (50) is coupled to the skin-application portion (40) and configured to be placed in acoustic contact with the skin, and configured to apply ultrasound energy to the skin. The apparatus (20) additionally comprises circuitry configured to generate a current responsive to motion of the skin-application portion (40); and a control unit (10), which is configured to receive the current, to determine, responsive thereto, whether the skin-application portion (40) is moving with respect to the skin, and to drive the acoustic element (50) to apply the ultrasound energy to the skin responsive to determining that the skin-application portion (40) is moving with respect to the skin.
