    • A system and method for inserting a gastrostomy catheter (10) through a passageway formed through the abdominal and stomach walls (11, 12) of a patient. The gastrostomy catheter (10) is mounted onto a dilatation catheter (13) with a dilatation member such as an inelastic balloon (14) on the distal extremity thereof. An introducer needle is first advanced through the walls of the patient's abdomen and stomach (11, 12) into the interior of the stomach and then a guidewire is advanced through the inner lumen of the needle into the stomach interior. The gastrostomy catheter-dilatation catheter assembly (10, 13) is advanced over the guidewire until the balloon (14) on the dilatation catheter (13) is in proper position crossing both the abdominal and gastric walls (11, 12). Upon inflation of the balloon (14) on the dilatation catheter (13), the passageway is expanded enough so that the gastrostomy catheter (10) can be advanced therethrough to dispose the distal end of the gastrostomy catheter (10) within the interior of the stomach. The balloon (14) on the distal end of the gastrostomy catheter (10) is inflated so as to form an internal retention member and the catheter withdrawn in order to urge the inflated balloon (14) against the stomach wall (12). Preferably, the gastrostomy catheter (10) has an external retention ring (15) on the shaft thereof which is slid against the exterior of the patient's abdomen to seal the passageway through the abdominal wall (11).

    • A surgical apparatus (10) includes a first instrument, such as an injection needle (12); a second instrument, such as a snare (14); and a catheter (16) for sheathing the first and second instrument. An actuator (18) is included for extending the first instrument (12), a substantially fixed first length relative to the distal end of the catheter (16); and for retracting the second instrument (14), a variable second length relative to the catheter (16) distal end. The first instrument (12) is biased into retracting relative to the catheter (16) distal end. Preferably, the actuator (18) has a support (40) and a slider (44). The slider (44) is slidably connected to the support (40) for movement in opposite directions relative thereto. The first instrument (12) is secured to the support (40) for movement therewith, and the second instrument (14) is secured to the slider (44) for movement therewith. A rail (50) enables independent but non-simultaneous sliding movement of the first instrument (12) and the second instrument in opposite directions, the rail (50) being longitudinally slideable with respect to the support (40), and operatively connected to the slider (44).
