    • A bone plug removal tool (30) is described. The tool (30) includes a cylindrical cutting element (32) having an external surface (34) and having an internal surface (36) defining an internal bore (38) extending along a longitudinal axis (40) of the cutting element (32) from a proximal end (42) to a distal end (43). A cutting tooth (44) is located at the proximal end (42) of the cutting element (32) and extends into the internal bore (38). A replaceable outer cylindrical sheath (48) is arranged concentrically around the external surface (34) of the cylindrical cutting element (32). A bone plug emplacement tool (80) to be used in conjunction with the removal tool (30) is also disclosed. The emplacement tool (80) includes a cylindrical element (82) having an internal surface (84) defining an internal bore (86) extending along a longitudinal axis (87) of the element (82) from a proximal end (88). The internal surface (84) further defines an internal step (90) adjacent a distal end (92) of the element (82). A stem is disposed for coaxial movement within the element (82), the stem (94) having a proximal end disposed within the internal bore (86). A shoulder (98) is defined in the stem (94) for mating engagement within the internal distal step (90) of the element (82) in order to limit distal movement of the stem (94) within the internal bore (86). A kit incorporating the various tools is disclosed. The kit, for repair of an articular cartilage (112), includes the bone plug removal tool (30) of the invention, an elongated plunger (70) for insertion into the proximal end (42) of the cutting element (32) and for coaxial movement within it, the plunger (70) for displacing a bone plug (78, 118) from the distal end (43) of cutting element (32) and the bone plug emplacement tool (80) of the invention. Kits may also include a drill bit containing a depth stop. A method of repairing a defective articular cartilage is also described.

    • A suture tensioning device (10) is described that comprises first (12) and second (14) jaws in facing relationship and oriented in parallel to each other. The first (12) and second (14) jaws are disposed in relative slidable engaement so that one of the jaws is adapted for parallel movement towards and away from the other jaw to define a variable gap (13) between them. A suture retainer (38) is engaged with a surface (30) of the second jaw (14) to enable a suture (20) to be tied so that the suture (20) exerts a variable tension on the surface (30) caused by the variable gap (13) between the first (12) and second (14) parallel jaws. Preferably, the first jaw (12) has defined therein a shape (42) designed to engage with a surface (16) from which a suture emanates. Preferably, the device includes two L-shaped elements (24, 25), each element having a shorter (28, 31) and a longer (26, 32) arm in orthogonal relationship. The longer arms (26, 32) of the respective support elements (24, 25) are slidably engaged with each other and adapted for movement from a first position, wherein the shorter arms (28, 31) are in registration and spaced apart from each other, to a second position, where the shorter arms (28, 31) are in registration and are closer together or farther apart than at the first position, so that a variable gap (13) between the shorter arms (28, 31) is defined. A suture retainer is engaged with one (28) of shorter arms and adapted for tying off a suture (20) emanating from a surface (16) and a conforming element (42) is engaged with the other arm (31). Methods for adjustably tensioning a suture (20) and a kit for performing suspension of a tissue (22) are described.

    • The present invention is directed to novel surgical systems that include a combination of an improved bone fastener, suture grasping device and/or suture throw rundown instrument. The systems can be used, e.g., for endoscopic procedures to repair and reattach soft tissues. The systems include a bone fastener comprising an expandable member having an axial channel and an elongated element inserted into the axial channel. The expandable member is configured to be insertible into a bore drilled in bone. The member is expanded using a continuous, compressive force (i.e., pressure without impulse or impact). The expandable member is grasped at its distal end throughout the emplacement procedure and is axially released from an emplacement tool. The systems can further include a suture grasping device comprising a rigid, hollow shaft, a rod, a first elongate wire-like element, a second elongated wire-like element, and an actuation device. The systems further include a suture throw rundown element comprising a handle assembly having first and second handle members movably connected for movement relative to one another, an elongate rod releasably secured to the first handle member.
