There is at least one fastener within the housing. The device also includes an anvil movable within the staple housing. The anvil has a proximal position and a distal position, wherein when in the distal position a distal end of the anvil extends distal to the distal end of the staple housing by a distance. The device also includes an adjustment nut connecting the staple housing to the handle. Rotation of the adjustment nut changes the distance.

摘要： Surgical instruments and techniques for using the instruments are provided for creating a pneumostoma through the chest wall into the lung of a patient. The pneumostomy instruments and techniques may be used to create a pneumostoma which allows gases to escape from the lung through the chest wall and thereby treat chronic obstructive pulmonary disease. The pneumostomy instrument includes a flexible shaft having an expanding pneumoplasty device at the distal end for displacing and engaging lung tissue. A mandrel is provided to facilitate introduction of the pneumoplasty device into the lung. A flange attached to the shaft allows the pneumoplasty device and lung to be drawn towards and the chest wall and secured in place during while the tract of the pneumostoma heals. The instrument is designed to be secured to the chest of the patient in a low-profile configuration during the post-operative period.

摘要： There is disclosed a porous emboli deflector for preventing cerebral emboli while maintaining cerebral blood flow during an endovascular or open surgical procedure. The device prevents the entrance of emboli of a size able to cause stroke (such as greater than 100 microns) from entering either the right or left common carotid arteries, and/or the right or left vertebral arteries by deflecting emboli downstream of these vessels. The device can be placed prior to any manipulation of the heart or aorta allowing maximal protection of the brain during the index procedure. The deflector has a low profile within the aorta which allows sheaths, catheters, or wires used in the index procedure to pass. The deflector shaft could have a first portion and a second portion, the second portion being more flexible than the first portion. The deflector frame can include one, two, or more movable structures, such as hinges. Also disclosed are methods for insertion and removal of the deflector.

摘要： Implantation devices for implanting antifungal compositions within one or more digits are described. The implantation devices may be configured so that they are capable of manipulation using no more than two digits of one hand of the user. The devices may also include improved safety features. For example, the devices may include an actuation mechanism that fully retracts the skin-penetrating conduit and stylet into the housing after the composition is implanted. Such an actuation mechanism may automatically initiate retraction of the stylet by retraction of the conduit. Implantation methods and kits comprising the devices are also described.

摘要： A flexible pneumostoma management device maintains the patency of a pneumostoma while controlling the flow of material through the pneumostoma. The pneumostoma management device includes a pneumostoma vent having a tube which enters the pneumostoma to allow gases to escape the lung, a flange and a filter/valve to control flow of materials through the tube. The flange is a thin flexible patch which conforms and attaches to the chest of the patient. The flange secures the tube in position in the pneumostoma. In alternate embodiments the pneumostoma management device comprises a vent tube and chest mount.

摘要： One aspect of the invention relates to an apparatus which has a light emitting portion for directing light radiation from the apparatus onto the lining of a body cavity for treating an ailment in a body cavity of a patient. For example, the ailment may be a gastrointestinal ailment of a patient, such as gastritis, gastric ulcer, duodenal ulcer, gastric cancer, gastric lymphoma, ulcerative colitis, or Crohn's disease. The apparatus may also be used for treating diseases of the circulatory system, urogenital systems and other body cavities. In one embodiment, the apparatus is inserted into a body cavity, e.g., stomach or colon, of the patient to place the distal tip of the apparatus in the desired position. A balloon is then inflated around the light-emitting portion of the apparatus, e.g., an array of optical fiber ends. The body cavity of the patient is then irradiated with light radiation so as to kill or debilitate microorganisms lining the body cavity without significant destruction of the body tissue of the patient, thereby improving or alleviating one or more of the symptoms associated with the ailment. Following treatment, a probiotic may be administered to the patient to reestablish the growth of normal microbial flora after treatment of the gastrointestinal tract.

摘要： One aspect of the invention relates to an apparatus which has a light emitting portion for directing light radiation from the apparatus onto the lining of a body cavity for treating an ailment in a body cavity of a patient.
