摘要： The present application describes an implant system useable for positioning an implant device such as a device useful for restricting passage of ingested food into the stomach. In one embodiment, the disclosed system includes a plurality of anchors that may be coupled to tissue within the stomach, or to a tissue tunnel formed by plicating stomach wall tissue. The anchor includes a loop. During use, the implant device is inserted through the loop and expanded such that it retains its position within the loop until removed. Instruments for implanting and explanting the implant device are also described.

摘要： Methods and apparatus for controlled grasping and cinching or locking of a tissue anchor are provided. In one variation, a tube is provided having a lumen and a resilient member that obstructs the lumen. A grasper may be advanced coaxially through the lumen, such that it reversibly displaces the resilient member and extends beyond the lumen's outlet to engage an element of the tissue anchor. The grasper then may be retracted within the tube, such that the resilient member again obstructs the lumen of the tube. Continued retraction of the grasper may act to cinch the anchor, for example, via interaction between the anchor and the obstructing resilient member. During cinching, a cinching mechanism of the anchor optionally may be positioned at least partially within the tube to enhance lateral stability. Furthermore, feedback indicative of a degree of cinching or locking may be provided during cinching.

摘要： A bone fastener including an expandable member (10) having an axial channel (18) and an elongated element (34), insertable into the axial channel (18), is described. The expandable member (10) is configured to be insertable into a bore drilled in a bone. The distal end (14) of the expandable member (10) also may include a structure for axially releasing the expandable member (10) from an emplacement device that places the expandable member (10) into a bone opening. In one embodiment the structure for axially releasing includes a frangible membrane capable of being severed from the expandable member (10) during emplacement of the expandable member (10) in a bone opening. A rivet (22) for coupling an object to bone for use with an expandable member (10), is also described. The distal end (14) of the elongated element (34) is shaped into a radially projecting portion adapted for engagement with a washer (62) that contacts the object to be coupled. The washer (62), having upper and lower surfaces, and a bore defined between the surfaces, is disposed around a portion of the shaft. The elongated element (10) is adapted for movement independent of the washer since the radial projection of the element has a different radius of curvature than the washer (62).

The all-suture anchor includes a fibrous construct which is movable between a pre- deployment configuration and a deployed configuration. The fibrous construct has a first density. The all-suture anchor also includes a monofilament woven through the fibrous construct. The monofilament has a second density, which is different than the first density. The contrasting densities (i.e., irregularities) aids in the "locking" ability of the all-suture anchor by greater purchase in hard and soft bone. The all-suture anchor can have a variety of textures, barbs, and rigidities that aid in creating irregularities within the bone surface for more secure fixation.

摘要： A system for deploying a suture implant includes a delivery device and a suture implant (100) coupled with the distal end of the delivery device. The suture implant includes a suture (108) and a slip knot (124) that defines a suture loop (122), the slip knot defining a dynamic end of the suture loop that is located opposite a closed end of the suture loop. The suture implant includes a tissue anchor (102) having first and second openings, whereby the closed end of the suture loop passes through the first and second openings for securing the tissue anchor to the suture loop. A tensioner (106) is secured to the first end (110) of the suture, and a pledget (104), located between the tensioner and the tissue anchor, is secured to the second end (116) of the suture. Pulling the tensioner away from the tissue anchor slides the pledget and the slip knot toward the tissue anchor for shortening the length of the suture loop and reducing the distance between the pledget and the tissue anchor.
