摘要： New methods for implanting a cardiac therapy system include implanting a lead of the system substernally anterior of the heart without attaching to the myocardium or pericardium. An illustration includes placement of an anchor beneath the sternum in the vicinity of one of the sternal angle, a location superior of the ventricles, the area bounded by the 2nd or 3rd ribs, and level with the aortic arch. A tension element or tether is attached to the anchor and a lead is introduced over the tension element or tether and secured in a desired position relative to the anchor. Other examples also include implantation, substernally, of a lead without the use of a pre-tunneling tool or sheath over the lead itself, for example by using an advancing tool for pushing the lead into position.

A control electronics subassembly defines the housing proximal end, and a battery subassembly defines the housing distal end. A distal fixation member extends from the housing distal end for fixing the housing distal end at an implant site. A pacing extension extends from the housing proximal end and carries a pacing cathode electrode. The pacing extension extends the pacing cathode electrode to a pacing site that is spaced apart from the implant site when the pacemaker is deployed in a patient's body.

摘要： Embodiments of the present invention generally relate to epicardial heart rhythm management. In some embodiments a method is provided for managing heart rhythm by implanting a pacing device within the pericardial space of a patient. In some embodiments, an implantable device is provided for pacing the epicardial surface of a patient. The device may include a first module coupled to a second module by a tether. The first and second modules may be selectively placed for bi-ventricular pacing or for dual chamber pacing in some embodiments. Optionally the modules may be separate and may wirelessly communicate with one another and may also communicate with a heart stimulation device programmer. Optionally, a plurality of devices may be implanted in the pericardial space. A delivery device for delivering the implantable device to the pericardial space is also provided. Further embodiments provide implant retrieval devices and methods.
