摘要： Systems and methods for performing surgical procedures on bone structures located within body tissue including an expandable conduit (22) comprising a wall portion (22) defining an internal passage therethrough and an initial configuration having a first cross-sectional area at a distal portion (24) thereof for percutaneous insertion into the body tissue. The wall portion (22) is movable against the body tissue to a second configuration having an enlarged cross-sectional area at the distal portion (22) thereof. An elongated member (650) and a first and second fastener (600) are configured for insertion into the internal passage. The first and second fasteners (600) are configured for fixation to the bone portion. The fastener (600) defines a recess for receiving the elongated member (650) therein. A driver member (660) is used to secure the clamping member (610) to the housing (604). The first and second bone portions are preferably first and second vertebrae.

摘要： Systems and methods for restructure and stabilization of bones that provide an antimicrobial effect are disclosed herein. A device includes a delivery catheter having an inner void for passing at least one light sensitive liquid, and an inner lumen; an expandable member releasably engaging the distal end of the delivery catheter; at least one channel positioned in the expandable member; and a light conducting fiber sized to pass through the inner lumen of the delivery catheter and into the expandable member, wherein, when the light conducting fiber is in the at least one channel, the light conducting fiber is able to disperse light energy to provide an anti-microbial effect. When the light conducting fiber is in the expandable member, the light conducting fiber is able to disperse the light energy to initiate hardening of the light sensitive liquid within the expandable member to form a photodynamic implant.

摘要： An artificial vertebral disc (100) for replacing an intervertebral disc of a patient is provided. The artificial disc (100) can comprise a first plate (110), a second plate (120), and an elongate member (140) extending between the first plate (110) and the second plate (120). The elongate member (140) is preferably flexible to allow angulation between the plates (110), (120). In one embodiment, a side support (130) extends along the exterior of the disc (100) to substantially enclose the disc (100). In another embodiment, the disc has an open configuration and a stopping member to limit the angulation of one plate relative to another plate. The disc can include an insert configured to be inserted in a space defined by the stopping member and one of the plates to prevent movement of the plates. Methods for replacing an intervertebral disc in an interbody space of a spine using the disclosed artificial discs are also provided.

摘要： Systems, devices, and methods suitable for use with procedures performed at least partially percutaneously are provided. In some procedures, two or more access devices for providing access to adjacent surgical locations within a patient are used. Certain embodiments of the access device comprise an elongate body having a distal end with one or more cutouts. The cutouts on adjacent access devices are generally aligned with each other to permit passage of a portion of a fixation element from one access device to the other access device. A fastener with an elongated removable head may be delivered to the surgical site through the access device. After a distal end of the fastener is secured to the surgical site, a portion of the elongated housing is detached from the remainder of the fastener and removed from the patient.

The elongate body defines a first passage (4066) for accessing the surgical location with surgical instruments. The device (4010) has a contracted configuration for insertion into the patient and an expanded configuration for providing access to the surgical location. The cross-sectional area of the first passage (4066) at a first location of the expanded configuration is greater than the cross-sectional area of the first passage (4066) at a second location of the expanded configuration. A second passage (4012) is separate from the first passage (4066) and is formed integrally with the elongate body. The second passage (4012) extends from the proximal portion (4032) toward the distal portion (4028) and is sized and configured to receive a viewing element for visualizing the surgical location.

摘要： A structure for receiving surgical instruments for performing a surgical procedure on a body includes a passage (16) through which the surgical instruments are inserted into the body. The passage (16) has a proximal end and a distal end. An expandable portion (40) enables an increase in the cross-sectional area of the distal end of the passage (16). The expandable portion (40) has a contracted condition in which the cross-sectional area of the distal end of the passage has a first cross-sectional area. The expandable portion (16) has an expanded condition in which the distal end of the passage has a second cross-sectional area greater than the first cross-sectional area. The second cross-sectional area is greater than a cross-sectional area of the proximal end of the passage when the expandable portion is in the expanded condition. A retaining mechanism (106) resists movement of the expandable portion (40) from the expanded condition toward the contracted condition during the surgical procedure. The retaining mechanism (106) is released at the conclusion of the surgical procedure to permit movement of the expandable portion from the expanded condition toward the contracted condition for removal of the structure.
