摘要： Systems, devices, and methods suitable for use with procedures performed at least partially percutaneously are provided. In some procedures, two or more access devices for providing access to adjacent surgical locations within a patient are used. Certain embodiments of the access device comprise an elongate body having a distal end with one or more cutouts. The cutouts on adjacent access devices are generally aligned with each other to permit passage of a portion of a fixation element from one access device to the other access device. A fastener with an elongated removable head may be delivered to the surgical site through the access device. After a distal end of the fastener is secured to the surgical site, a portion of the elongated housing is detached from the remainder of the fastener and removed from the patient.

摘要： An interactive, graphical depiction of a form ( e.g ., a tax, banking, insurance or other form) is presented through a computer network. A request is received through the network for information relating to a location on the form selected by a user, where the selection is made through interaction with the graphical depiction of the form. Information is delivered to the user through the network in response to the request. The delivered information may include one or more of the following relating to the selected form location: planning information, consulting materials, research and compliance information. An interactive depiction of a map or series of maps also may be presented through the network. The user may interact with the map to obtain jurisdiction-specific information on a topic that relates to the subject matter of the graphical, interactive form. The jurisdiction-specific information also may be delivered to the user through the network.

摘要： Systems and methods for performing surgical procedures on bone structures located within body tissue including an expandable conduit (22) comprising a wall portion (22) defining an internal passage therethrough and an initial configuration having a first cross-sectional area at a distal portion (24) thereof for percutaneous insertion into the body tissue. The wall portion (22) is movable against the body tissue to a second configuration having an enlarged cross-sectional area at the distal portion (22) thereof. An elongated member (650) and a first and second fastener (600) are configured for insertion into the internal passage. The first and second fasteners (600) are configured for fixation to the bone portion. The fastener (600) defines a recess for receiving the elongated member (650) therein. A driver member (660) is used to secure the clamping member (610) to the housing (604). The first and second bone portions are preferably first and second vertebrae.

摘要： A spud assembly for use with a fuel tank having a body defining an interior chamber and having an outer periphery. The spud assembly includes a base attached to the outer periphery of the fuel tank and the base has a throughbore open to both the interior chamber of the tank as well as exteriorly of the tank. An elongated spud having an axial throughbore is movable relative to the base between a connected and an unconnected position. In its connected position, the spud is positioned through the base throughbore so that the spud fluidly connects the interior of the fuel tank to the spud. Additionally, when the spud is moved to its fully connected position, a locking finger on the base engages a lock abutment surface on the spud to secure the spud and base together. In an alternate embodiment, a locking ring is coaxially disposed over the spud and base when the spud is in its connected position. The locking ring is rotatable between an unlocked position in which the spud and base can be disconnected from each other, and a locked position in which the spud and base are secured together.

摘要： A spud assembly (10) for use with a fuel tank (12) having a body (14) defining an interior chamber (16) and having an outer periphery (18). The spud assembly (10) includes a base (20) attached to the outer periphery (18) of the fuel tank (12) and the base (20) has a throughbore (26) open to both the interior chamber (16) of the tank (12) as well as exteriorly of the tank. An elongated spud (40) having an axial throughbore (42) is movable relative to the base (20) between a connected and an unconnected position. In its connected position, the spud (40) is positioned through the base throughbore (26) so that the spud (40) fluidly connects the interior of the fuel tank (12) to the spud (40). Additionally, when the spud (40) is moved to its fully connected position, a locking finger (128) on the base (20) engages a lock abutment surface (156) on the spud (40) to secure the spud (40) and base (20) together. In an alternate embodiment, a locking ring (62) is coaxially disposed over the spud (40) and base (20) when the spud (40) is in its connected position. The locking ring (62) is rotatable between an unlocked position in which the spud (40) and base (20) can be disconnected from each other, and a locked position in which the spud (40) and base (20) are secured together.
