摘要： One embodiment is directed to a system for providing surgical access across a wall of a tissue structure, comprising a delivery member, a first helical member, an anchor member removably coupled to the helical member distal end, and a suture member coupled distally to a portion of the anchor member and extending proximally to a position wherein at least a portion of it may be freely manipulated by an operator; wherein upon rotation of the delivery member in a first direction, the first helical member and coupled anchor member are advanced across at least a portion of the wall of the tissue structure, pulling along the distal portion of the suture member in a deployed suture pattern which remains coupled to the anchor member, the deployed suture pattern being characterized in that it is substantially helical with between about one and three helical loops.

摘要： A device for providing access to a surgical location within a patient is provided. The device includes an elongate body (11004, 12004, 13004) and a sleeve (11006, 12006, 13006). The elongate body (11004, 12004, 13004) has an outer surface (11008, 12008, 13008), an inner surface (11012, 12012, 13012), and a distal portion (11016, 12016, 13016). The inner surface (11012, 12012, 13012) defines a passage (11020, 12020, 13020) that extends through the elongate body (11004, 12004, 13004) along a longitudinal axis (11024, 12024, 13024) through which surgical instruments can be inserted to the surgical location. The distal portion (11016, 12016, 13016) is capable of having an expanded configuration when inserted within the patient. In the expanded configuration, the cross-sectional area of the passage (11020, 12020, 13020) at a first location (11028, 12028, 13028) is greater than the cross-sectional area of the passage (11020, 12020, 13020) at a second location (11032, 12032, 13032), wherein the first location (11028, 12028, 13028) is distal to the second location (11032, 12032, 13032). 摘要： A spacer device is provided for use with a primary spinal fixation device to treat, reduce, or delay adjacent level degenerative disc disease. The spacer device comprises a compressible spacer, a transverse member, and a connecting member. The compressible spacer is sized to fit between the spinous processes of two adjacent vertebrae and is configured to reduce the range of motion of at least one vertebra. The transverse member is configured to extend from one side of the midline of the spine, extending through the interspinous process space. The transverse member is coupled with the spacer. The connecting member is attachable to the transverse member and to a primary spinal fixation device.

摘要： In one embodiment a device for retracting tissue provides access to a spinal location within a patient. The device has an elongate body that has a proximal end and a distal end. The elongate body has a length between the proximal and distal ends such that the distal end can be positioned inside the patient adjacent the spinal location. The elongate body has a generally oval shaped proximal portion and an expandable distal portion. A passage extends through the elongate body between the proximal and distal ends. The passage is defined by a smooth metal inner surface extending substantially entirely around the perimeter of the passage between the proximal and distal ends. The elongate body is expandable between a first configuration sized for insertion into the patient and a second configuration wherein the cross-sectional area of the passage at the distal end is greater than the cross-sectional area of the passage at the proximal end.

摘要： Systems and methods for performing surgical procedures on bone structures located within body tissue including an expandable conduit (22) comprising a wall portion (22) defining an internal passage therethrough and an initial configuration having a first cross-sectional area at a distal portion (24) thereof for percutaneous insertion into the body tissue. The wall portion (22) is movable against the body tissue to a second configuration having an enlarged cross-sectional area at the distal portion (22) thereof. The apparatus includes a fastener engageable with a bone portion, a housing having a first passage configured to receive a longitudinal member and a second passage configured to receive the fastener. The fastener extends through an opening in the housing into the second passage and is movable relative to the housing. The longitudinal axis of the fastener is positionable in any one of a plurality of angular positions relative to a longitudinal axis of the second passage. The apparatus also includes a clamping member configured to be received in threaded engagement within the housing. The clamping member has a first end configured such that when a longitudinal member is positioned in the first passage in the housing, the first end of the clamping member engages the longitudinal member at an angle not orthogonal to an axis along which the clamping member is advanced.
