    • Methods and instrumentation for surgical procedures relating to preparation of vertebral bodies and a disc space for insertion of spinal implants is disclosed. The instruments and methods are particularly adapted for a posterior approach to the spine. One instrument is a shim (10) having a blade (14) and a shaft (12) having a first end connected with the blade (14). The blade (14) is inserted into a distracted disc space to maintain distraction during subsequent procedures. In one embodiment, the shaft (12) can be bent away from the operative field. In another embodiment, the shim (10) is used in an endoscopically assisted approach to the spine. Another instrument is a driver for driving the blade (14) of the shim (10) into the distracted disc space. Various methods contemplate the use of these instruments to maintain disc space distraction during disc space preparation for receiving an implant.

    • A posterior fixation system includes a saddle member, and anchoring member, an occipital plate, an occipital rod, and a cross-link connector. The anchoring member anchors the saddle member to bone. The saddle member includes a pair of upright portions that define a channel. The channel is adapted to receive an orthopedic rod, and the saddle member can include a hole to receive the anchoring member. The saddle member and the anchoring member can be coupled so as to allow multi-axial movement of the members. The anchoring member in one embodiment is a screw coupled to the hole of the saddle, and in another embodiment, the anchoring member is a hook. The offset member may be coupled to the saddle member to allow for offset connection of rods. Connection of individual rods can be accomplished by connecting the rods with the cross-link connector. The cross-link connector has an integrally formed cylindrical member that couples a pair of coupling portions together. The cylindrical member can be bent along multiple axes. The occipital plate secures the rods to the occipital bone of the skull. The occipital plate has a cross-shaped plate with a plurality of apertures defined in the plate and at least one saddle member coupled to the plate. Alternatively, the occipital rod can be secured to the occipital bone.

    • A device and method of attaching an elongated rod (50) to different vertebral members to form a spinal fixation system. The device comprises a securement member (20) having a first section (22) that is positioned within the vertebral member. A second section (24) extends laterally outward from the first section (22). A receiving member (30) is mounted to the second section (24) to receive the elongated rod (50). Receiving member (30) includes a receiver (33) for receiving and attaching to the second section (24). The receiving member (30) is laterally movable along the lateral section (24) for positioning the elongated rod (50) relative to the spine of the patient. The length of the second section (24) provides for mounting the first section (22) at a variety of anatomically conducive locations on the vertebral member while supporting the elongated rod (50).
