摘要： Systems, devices, and methods suitable for use with procedures performed at least partially percutaneously are provided. In some procedures, two or more access devices for providing access to adjacent surgical locations within a patient are used. Certain embodiments of the access device comprise an elongate body having a distal end with one or more cutouts. The cutouts on adjacent access devices are generally aligned with each other to permit passage of a portion of a fixation element from one access device to the other access device. A fastener with an elongated removable head may be delivered to the surgical site through the access device. After a distal end of the fastener is secured to the surgical site, a portion of the elongated housing is detached from the remainder of the fastener and removed from the patient.

摘要： An artificial vertebral disc (100) for replacing an intervertebral disc of a patient is provided. The artificial disc (100) can comprise a first plate (110), a second plate (120), and an elongate member (140) extending between the first plate (110) and the second plate (120). The elongate member (140) is preferably flexible to allow angulation between the plates (110), (120). In one embodiment, a side support (130) extends along the exterior of the disc (100) to substantially enclose the disc (100). In another embodiment, the disc has an open configuration and a stopping member to limit the angulation of one plate relative to another plate. The disc can include an insert configured to be inserted in a space defined by the stopping member and one of the plates to prevent movement of the plates. Methods for replacing an intervertebral disc in an interbody space of a spine using the disclosed artificial discs are also provided.

摘要： A system for providing access to a surgical location adjacent to a spine of a patient is provided. The system includes a dilator and an access device. The dilator has a proximal portion, a first elongate member, a second elongate member, and a movable member at least partially located between the first and second elongate members. The proximal portion is coupled with and is configured to move the movable member to move the dilator to an expanded configuration. The access device has a passage that extends therethrough. The passage is configured such that the access device can be advanced over the dilator when the dilator is in the expanded configuration.

摘要： A retractor (4010) is provided that has a proximal portion (4022) that comprises a first side portion (4054) having a first longitudinal edge and a second side portion (4058) having a second longitudinal edge. The first and second portions are movable relative to each other such that the first and second longitudinal edges can be positioned in close proximity to each other or spaced apart by a selected distance. 摘要： In one embodiment a device (5000) for retracting tissue provides access to a spinal location within a patient. The device has an elongate body (5002) that has a proximal end and a distal end. The elongate body has a length between the proximal and distal ends such that the distal end can be positioned inside the patient adjacent the spinal location. The elongate body has a generally oval shaped proximal portion (5010) and an expandable distal portion (5012). A passage (5014) extends through the elongate body between the proximal and distal ends. The passage is defined by a smooth metal inner surface extending substantially entirely around the perimeter of the passage between the proximal and distal ends. The elongate body is expandable between a first configuration (5052, 5054) sized for insertion into the patient and a second configuration (5056, 5058) wherein the cross-sectional area of the passage at the distal end is greater than the cross-sectional area of the passage at the proximal end.

摘要： One embodiment is directed to a system for providing surgical access across a wall of a tissue structure, comprising a delivery member ( 14 ), a first helical member ( 66 ), an anchor member ( 54 ) removably coupled to the helical member distal end, and a suture member ( 52 ) coupled distally to a portion of the anchor member and extending proximally to a position wherein at least a portion of it may be freely manipulated by an operator; wherein upon rotation of the delivery member in a first direction, the first helical member and coupled anchor member are advanced across at least a portion of the wall of the tissue structure, pulling along the distal portion of the suture member in a deployed suture pattern which remains coupled to the anchor member, the deployed suture pattern being characterized in that it is substantially helical with between about one and three helical loops.
