摘要： A biomedical material for transplant to a subject is provided according to embodiments of the present invention which includes an isolated donor tissue enzyme-treated to reduce the amount of proteoglycans in the donor tissue compared to untreated tissue. Isolated cells are optionally added to the enzyme-treated donor tissue, including leukocytes, particularly monocytes; macrophages; platelets; cells derived from an intervertebral disc such as chondrocyte-like nucleus pulposus cells; fibrocytes; fibroblasts; mesenchymal stem cells; mesenchymal precursor cells; chondrocytes; or a combination of any of these. The isolated donor tissue is articular cartilage or an intervertebral disc tissue such as nucleus pulposus tissue and/or annulus fibrosis tissue enzyme-treated to remove proteoglycans normally present in these tissues. A biomedical material of the present invention is administered to a subject to treat a disorder or injury, such as a disorder or injury to connective tissue.

摘要： The invention broadly facilitates reconstruction of the Annulus Fibrosus (AF) or the AF and the Nucleus Pulposus (NP). Such Reconstruction prevents recurrent herniation following Microlumbar Discectomy (MLD) other procedures. The invention may also be used in the treatment of herniated discs, annular tears of the disc, or disc degeneration, while enabling surgeons to preserve the contained NP. The methods and apparatus may be used to treat discs throughout the spine including the cervical, thoracic, and lumbar spines of humans and animals. In the preferred embodiment, a spinal repair system according to the invention comprises a first end portion adapted for placement within an intervertebral body, a second end portion adapted for placement within an adjacent intervertebral body, and a bridge portion connecting the first and second end portions, the bridge portion being adapted to span a portion of an intervertebral disc space and prevent excessive outward bulging.

摘要： This invention improves upon prior aft total disc replacements (TDRs) by more closely replicating the kinematics of a natural disc. The preferred embodiments feature two or more fixed centers of rotation (CORs) and an optional variable COR (VCOR) as the artificial disk replacement (ADR) translates from a fixed posterior COR that lies posterior to the COR of the TDR to facilitate normal disc motion. The use of two or more CORs allows more flexion and more extension than permitted by the facet joints and the artificial facet (AF). AF joint-like components may also be incorporated into the design to restrict excessive translation, rotation, and/or lateral bending.

摘要： Existing bone dowels are improved through the use of rigid, preferably metallic end plates (306, 320). The plates are positioned relative to the anterior and posterior end surfaces of the bone dowel. The completed structure may be inserted, removed, and positioned into the vertebrae. The use of the rigid endplates (306, 320) facilitates load sharing which helps to prevent graft fracture. In the preferred embodiment, the end plates (306, 320) are positioned against the end surfaces of the bone section (400) through the use of a link member (308), which passes through the interior of the bone section (400) and connects the end plates (306, 320). One of the end plates (320) may include a threaded bore, and one end of the link member may be threaded to receive the threaded bore of that end plate to hold it in place.

摘要： Apparatus, systems and methods are used to repair, reconstruct and/or replace spinal features. A flexible longitudinal fixation component such a suture is passed around or through a portion of a spine, with the end sections of the component either being attached to one another or coupled to at least one bone implant. The bone implant may be a bone anchor or may form part of a pedicle screw assembly. The flexible longitudinal fixation component may pass through an anulus fibrosis (AF) and at least one intra-aperture component situated within a void or defect in an AF. The intra-aperture component may be composed of a porous mesh, allograft tissue or xenograft tissue. The intra-aperture component preferably includes one or more proximal-to-distal channels facilitating the intentional initial passage of nucleus pulposis (NP) tissue while preventing the extrusion of the NP long term.

摘要： Apparatus and methods facilitates reconstruction of the anulus fibrosus (AF) and/or the nucleus pulposus (NP) to prevent recurrent herniation following microlumbar discectomy. The invention may also be used in the treatment of herniated discs, anular tears of the disc, or disc degeneration, while enabling surgeons to preserve the contained nucleus pulposus. A spinal repair system according to the invention comprises flexible longitudinal fixation components adapted for placement through portions of the AF with intact fibers, a porous mesh reinforcement component adapted for placement over a region of the AF with damaged fibers, and an anti-adhesion component for placement over flexible longitudinal fixation components and the porous mesh component. Preferred embodiments of the invention include an intra- aperture component dimensioned for positioning within a defect in the AF, with one or more components being used to maintain the intra- aperture component in position. One or more lengthwise passageways through the intra-aperture component, one or more lengthwise grooves on the outer surface of the intra-aperture component, or a combination thereof, intentionally facilitate the escape of nucleus pulposus tissue through or around the intra-aperture component in response to pressure applied by the upper and lower vertebral bodies.

摘要： Devices and methods for fixing defects in the anulus fibrosus (vertebral disc) of a patient are described. The devices a mesh patch; first, second, third, and fourth sutures; and first, second, third, and fourth anchors. Each anchor has a first portion adapted for insertion into a bone and a second portion having an opening. The sutures are disposed through the openings of the anchors. The first portions of the first and second anchors are inserted into a cranial vertebra. The first portions of the third and fourth anchors are inserted into a caudal vertebra. The mesh patch is positioned adjacent the defect. An end of first suture can be attached to an end of the third suture. An end of the second suture can be attached to an end of the fourth suture. The other ends of each of the first, second, third, and fourth sutures can then be anchored.
