    • A cardiac rhythm management system includes techniques for computing an indicated pacing interval, AV delay, or other timing interval. In one embodiment, a variable indicated pacing interval is computed based at least in part on an underlying intrinsic heart rate. The indicated pacing interval is used to time the delivery of biventricular coordination therapy even when ventricular heart rates are irregular, such as in the presence of atrial fibrillation. In another embodiment, a variable filter indicated AV interval is computed based at least in part on an underlying intrinsic AV interval. The indicated AV interval is used to time the delivery of atrial tracking biventricular coordination therapy when atrial heart rhythms are not arrhythmic. Other indicated timing intervals may be similarly determined. The indicated pacing interval, AV delay, or other timing interval can also be used in combination with a sensor indicated rate indicator.

The system includes an A wave detector (53, 55) for detecting A waves of the heart. The A wave detector has a first threshold, and a selected second threshold higher than the first threshold. An R wave detector (52) for detecting R waves of the heart is coupled to the A wave detector for selecting the second threshold of the A wave detector during each detected R wave of the heart. An atrial cycle length determining stage (64) determines atrial cycle length, and a comparator activates atrial fibrillation detection when an atrial cycle length is shorter than a predetermined cycle length.

    • The present invention is a device for preventing clotting during atrial fibrillation. The device of the present invention includes optional electrical cardiac output forcing (ECOF) or ICD/AED back-up. The device includes a power supply (40) for operating the device, and for providing the necessary output forcing signals. The power supply is located in a housing which may be implanted into a human patient. In one embodiment of the invention, the housing acts as an electrode. At least one additional electrode is connected to the housing, and may be inserted into the human heart. A ventricular lead (18) is mounted to the electrode along with an atrial lead (14). The housing also contains an electrical cardiac stimulator (34). When a certain cardiac event, such as atrial fibrillation, is detected, anti-clotting pulses are applied to the heart via the atrial lead. If, and when ventricular fibrillation is detected, further pulses of a different voltage level are applied to the heart via the ventricular lead. In one embodiment of the invention, the anti-clotting pulses are synchronized to the R-wave sensed by the ventricular lead.
