    • An adjustable retractor is disclosed comprising an inner ring, an outer ring spaced from the inner ring, and an elongate sleeve open at opposite ends. The sleeve is provided and extends between the inner and outer rings and is connected at the opposite ends to the rings. The outer ring is provided with a pre-loaded rotational torque to assist with rotation of the outer ring about its central axis to roll the sleeve about the outer ring to adjust sleeve length. The outer ring may be formed in a mobius configuration to provide the pre-loaded rotational torque. The outer ring has a cross-sectional shape that may have numerous different configurations. The mobius configuration may include the cross-sectional shape having an outer circumference offset by a predetermined amount about the central axis of the outer ring proceeding longitudinally along the central axis from one location on the outer ring to another location on the outer ring. A method of adjusting a retractor is also disclosed.

    • A retractor and a surgical tool are positioned within a cannula, and a dissection cradle of the retractor is positioned at the distal end of the cannula. The retractor includes a first portion with an axis approximately parallel to the axis of the cannula and a second portion with an axis skewed relative to the axis of the cannula. The dissection cradle may include two substantially parallel, spaced legs with the retractor shaped in a loop between and in a plane skewed relative to the axes of the legs, and with the loop directed away from the surgical tool. Thus, in operation, when a surgeon locates a vessel and side branch of interest, the surgeon extends the retractor to cradle the vessel in the dissection cradle. Once cradled, the retractor may be fully extended to urge the vessel away from the axis of the cannula to isolate the side branch for exposure to a surgical tool. One of the legs supporting the retractor is hollow and includes a spray nozzle disposed in the distal end of the retractor to form an irrigation system The proximal end of the hollow leg communicates with a fluid inlet which receives irrigation fluid under pressure for washing the endoscope lens. The spray nozzle on the retractor may be positioned to wash the endoscope lens, or upon selective extension of the retractor out of the cannula, the spray nozzle may be positioned to direct the spray of irrigation fluid at a remote surgical site.

    • In accordance with the present invention, there is disclosed surgical methods and apparatus for accessing and stabilizing the heart. The methods and apparatus facilitate access to an anastomosis site, allows various instruments or devices to be maneuvered and secured in place, and provide stabilization of the heart. In particular, the apparatus involves a retractor assembly having a pair of opposing blades having a channel adapted to engage an incision in a patient. The retractor blades may have features to cooperatively engage an instrument mount. The instrument mount may hold an instrument, such as a tissue stabilizer, and allows the instrument to be easily maneuvered. The retractor blades may have a number of suture locks for securing sutures used during surgery. The retractor system is particularly useful in accessing, positioning and stabilizing the beating heart for coronary artery bypass graft surgery.

    • A stabilized cephalic medical apparatus includes an annular band for placement around a patient's head, and a pivotally adjustable bridge member for holding and supporting a medical instrument. The bridge member according to the invention is attachable to the annular band, and is preferably removable therefrom for cleaning and sterilization thereof. The bridge member is preferred to include a mounting member for supporting a medical instrument thereon, and a connector for connecting the mounting member to the annular band. The apparatus may further include a medical instrument attachable to the bridge member. The design of the apparatus according to the invention allows a medical instrument, such as a nasal speculum or retractor, to be operatively attached to, and to move concurrently with the patient. Attachment of the medical instrument to a patient tends to minimize or eliminate complications arising from movement of a patient's head with respect to a fixed in place speculum, retractor, or other medical instrument. A method of using the apparatus in connection with a medical instrument is also described.

    • An inflatable retraction apparatus is provided for retracing an organ inside a body to gain access to an adjacent tissue. The apparatus includes an able chamber formed from a single thin envelope, and capable of being collapsed for insertion through an incision and being expanded to a predetermined shape for retracing the organ. An inflation mechanism is adapted to selectively inflate the chamber to the predetermined shape. The inflation mechanism includes at least one inflation tube attached to the inflatable chamber in fluid communication theretween, and an inflatable maintaining lattice operatively enveloping the inflatable chamber and in operative association with the inflation mechanism.
