    • An endoscopic suctioning instrument with variable suction is disclosed. The instrument has a hollow cannula having a proximal end and a distal end, and sliding sleeve having a proximal end and a distal end and a longitudinal axis, with the sliding sleeve being coaxial to and closely surrounding and slidingly engaging the hollow cannula. The sliding sleeve has a plurality of lateral holes at its distal end with the lateral holes being spaced along the longitudinal axis. A valve is provided for selectively coupling a suction source to the proximal end of the cannula, and a sleeve actuation mechanism is provided for sliding the sliding sleeve from a first position where the distal end of the sliding sleeve extends beyond the distal end of the cannula such that a first number of the lateral holes are unblocked by said cannula, to a second position where the distal end of the sliding sleeve extends beyond said distal end of the cannula such that fewer lateral holes are unblocked by said cannula, to a third position where the cannula blocks all of said lateral holes of the sliding sleeve. Various embodiments are described with different mechanisms for causing the sleeve to slide, including mechanisms linking the valve to the sleeve.

    • An apparatus for counting the number of times a temperature cycle occurs includes an indicator having sequential indicia and a temperature responsive member for indicating a next one of the indicia. Mechanical, fluid mechanical and electronic versions of the invention are disclosed. In the mechanical version, the indicator can be a ratchet member having teeth, a pawl and a display surface containing indicia. A temperature responsive element such as a bimetallic member engages the teeth of the ratchet and advances the ratchet each time the apparatus is subjected to a preset temperature cycle. A housing with a window masks the display to indicate one of the indicia as advanced by the temperature responsive element. Preferred embodiments of the mechanical version include a ring-like ratchet member with interior teeth and an exterior display surface. The number of teeth is preferably one or more less than the number of indicia so that upon advancing the ratchet member to the last indicia, it can be advanced no further. The bimetallic element is mounted inside the ring and the housing covers the outer surface of the ring. A removable locking pin is provided to prevent movement of the ratchet member until activated.

    • An endoscopic tool for dispensing units of hemostatic agent broadly comprises a hollow tube which holds a plurality of individual units of the hemostatic agent, a valve at the distal end of the hollow tube which permits the hemostatic agent to pass therethrough but prevents foreign matter from contacting the hemostatic agent while in the tube, a plunger which extends into the hollow tube and contacts a proximal unit of the hemostatic agent, and a mechanism for moving the plunger distally in incremental movements to cause, upon each incremental movement, an individual unit of the hemostatic agent to be pushed through the valve. Typically, the mechanism for moving the plunger and the proximal end of the hollow tube are held in a handle. One mechanism for moving the plunger utilizes a portion of the plunger as a ratchet by forming a plurality of conical elements thereon, and utilizes a switch in the handle which has a V-spring coupled thereto as a pawl for advancing the conical elements. A second mechanism utilizes a regularly shaped notched cutout in the handle as the ratchet, and a vertical protrusion on the plunger which extends through the cutout as an engagement element. The notched cutout provides resilient stops, and the protrusion is shaped to allow it to be forced in a forward direction through each resilient stop and then to sit in another notch.

    • An apparatus for counting the number of times a medical instrument has been sterilized includes an indicator having sequential indicia and a heat responsive member for indicating a next one of the indicia. Mechanical, fluid mechanical and electronic versions of the invention are disclosed. The housing may be formed from the ferrule of an endoscopic surgical instrument or may be formed as part of the handle of such an instrument. A removable locking pin is provided to prevent movement of the ratchet member during shipping and the locking pin may be coupled to packaging so that it is automatically removed when the surgical instrument is removed from its packaging.

    • A probe for use with an endoscopic suction-irrigation instrument is disclosed which permits a second probe to be inserted therethrough. The suction-irrigation instrument has a cannula, a fluid chamber having at least one port for irrigation and suction and a proximal opening, and a first seal in the proximal end of the fluid chamber. The fluid chamber has a distal end communicating with the cannula. The probe has a proximal handle portion, a middle portion, and a hollow distal portion. The handle portion has an axial passageway and a second seal therein. The middle portion extends from the handle portion through the first seal at the end of the fluid chamber and through the fluid chamber. The middle portion has an outer diameter shaped to contact the first seal such that the proximal end of the fluid chamber is sealed. The middle portion is also shaped to be smaller than the inner diameter of the fluid chamber, and the middle portion is provided with a second axial internal passageway extending therethrough. The distal portion extends at least partially through the cannula and has an outer diameter smaller than the inner diameter of the cannula. A second probe may be inserted through the second seal of the first probe, through the axial passageway in the handle portion, through the internal passageway in the middle portion, and out through the distal portion and the cannula.
