    • A medical apparatus and method of use for implanting a catheter in a patient's body which catheter can be easily positioned, repositioned, and replaced. The apparatus includes an elongate sleeve comprising a wall surrounding an interior passageway extending from a sleeve proximal end to a sleeve distal end. The sleeve is intended to be percutaneously implanted through an incision in the patient's skin to place the sleeve distal end subcutaneously. The sleeve distal end outer surface carries porous material intended to contact the patient's dermis to promote tissue ingrowth. The sleeve passageway is dimensioned to snugly accommodate the outer surface of the catheter while permitting the catheter to slide relative to the sleeve. A split ring locking member is selectively operable in an unlocked state for permitting axial and rotational movement of said catheter relative to said sleeve and operable in a locked state for preventing such relative movement.

    • An implantable device including a housing having a stud projecting percutaneously through an incision in the patient's skin. The stud defines a peripheral surface extending longitudinally inwardly from the stud outer end. The longitudinal peripheral surface is used as a substrate to carry a peripheral fibrous layer which extends longitudinally along the stud from its outer end adjacent to the skin incision to below the patient's epidermal and dermal skin layers for promoting laterally directed soft tissue ingrowth. The housing also defines a lateral shoulder surface oriented substantially perpendicular to the longitudinal peripheral surface. The shoulder surface also carries a porous layer conducive to promoting tissue ingrowth. The porous layers on the device allows tissue ingrowth into the interstices of both porous layers to form an enhanced infection resistant barrier while also providing improved device anchoring.

    • An electrokinetic delivery or diagnostic device for personal use and self-administration of a medicament to a treatment site is provided. The device includes a housing containing a power supply, a first active electrode and a ground tactile electrode. An applicator includes a pad for containing medicament for overlying the first electrode of the device. The medicament may be applied by the user to the pad, contained within the pad or contained in rupturable capsules within the pad, together with a hydrogel if necessary for imparting electrical conductivity whereby, upon application of the applicator to the treatment site, the medicament is electrokinetically transported into the treatment site. In another form, the applicator is self-contained, having a power supply, a first electrode overlying a pad and a ground electrode from the opposite side from the pad whereby the individual presses the pad against the treatment site to complete the electrical circuit and electrokinetically drive the medicament into the treatment site.

    • A urethral valve system includes a fixation balloon that is positioned in and contoured to the geometry of the bladder near the opening to the urethra. The fixation balloon, which is narrower at an end that is closest to the urethra and wider at an opposite end, includes a channel through which urine is directed to flow from the bladder to the urethra. A valve balloon is positioned in the channel and inflates to prevent urine flow through the channel and deflates to allow urine to flow around the balloon and out the end of the channel. The fixation balloon may be attached to a Foley-type catheter, with the catheter providing the channel through the balloon. The fixation balloon then acts as a shock absorber for the system by moving relative to the catheter in response to, for example, a contraction in the bladder. The catheter may contain lumens that provide access to the valve balloon and/or the fixation balloon. The fixation balloon is inserted in a deflated state and inflated to hold the system in place. The fixation balloon remains inflated with the valve balloon providing continence when it is in an inflated state and allowing voiding when it is in a deflated state. Only when the device is to be removed from the body is the fixation balloon deflated.

    • A urethral valve system includes a valve balloon that inflates to provide continence and deflates to allow urine to flow around the balloon and an attached catheter that extends through the urethra, and out of the body through the meatus. The urine cleanses the valve balloon, the catheter and the urethra of bacteria, and thus, minimizes infection. The catheter includes a check valve at the end that is accessible to the user. To inflate the balloon, the user attaches to the check valve a small pump that supplies air or a liquid such as water to inflate the balloon. To deflate the balloon, the user manipulates the check valve to allow the water or air to drain from the balloon. The valve balloon may be positioned in the bladder. To retain the system in the bladder when urine flows around the balloon and the catheter, the balloon deflates to a shape that is wider than the diameter of the neck of the bladder. Alternatively, the system may include an anchoring device, such as rubber fingers that extend from the base of the balloon beyond the neck of the bladder. The valve balloon may also be positioned in the urethra. The system then includes an anchoring device that is positioned in the bladder to hold the valve balloon in its deflated state in the urethra against the flow of urine through the urethra and around the balloon and the attached catheter. The valve system may further include an everting mechanism that provides non-irritating delivery of the valve balloon and/or the anchoring device.
