SOLUTION: A form device is to mold a molded article by concrete into a form comprising an outer form plate 11 (11A, 11B) and an inner form plate 12 (12A, 12B, 12C). Guide means are installed for allowing the outer form plate 11 and inner form plate 12 to freely rotate at a predetermined guide locus. In the guide means for the outer form plate 11, a guide groove 18 is formed in a semicircular guide plate 16 and bearings 17, 17 of the outer form plate 11B are inserted therein. On the other hand, in the guide means for the inner form plate 12, a guide groove 35 is formed in a guide plate 33 and bearings 90, 90 of the inner form plate 12B are inserted therein. In the case when the outer form plate is rotated, the end periphery thereof is in contact with the end periphery of the outer form plate 11A at all times, and in the case when the inner form plate 12B, the end periphery thereof is always in contact with the end periphery of the inner form plate 12. Accordingly, the outer form plates 11A, 11B and the inner form plates 12B, 12C create no interstices therebetween, thereby precluding concrete from coming out therefrom.

Since the lock part 30 is provided with a screw part 34 in the peripheral surface thereof, the lock part 30 adapts to the side wall, and the lock part 30 can be strongly embedded in the side wall. Since the screw part is provided, the lock part can be easily removed from the side wall. This hoisting jig can be recycled.
