The major diameter internal thread part 42 of the fixing nut member 4 is screwed and fixed to an external thread part 32 of the shaft lever 3, the locking member 5 is fixed to a shaft end surface 32a of the shaft lever 3 by a first fixing tool 71, the attachment tool 6 is fixed to the locking member 5 by welding while engaging locking the locking part 63 of the attachment tool 6 with the locked part 43 of the fixing nut member 4 and storing the locking member 5 in the joining opening part 62, and the attachment tool 6 is fixed to the fixing nut member 4 by a second fixing tool 72. This operation is then repeated to gradually discharge the material after cutting from the lower part of the through-hole 2.
