    • A catheter and patch electrode system (10) for use with an apparatus (e.g., an ablation generator) having a 4-wire interface (62) for improved impedance measurements which includes two source connectors for receiving excitation signals and a pair of sense connector wires for measuring impedances. A radiofrequency ablation generator (16) can produce a signal between a source wire and an indifferent return patch electrode (37). A source lead (46) is electrically coupled to a tip electrode (28T) and extends through to a proximal end (and an optional sense lead can couple the tip electrode as well). A source return (56) and a sense return (56) (e.g., skin patch(es)) can be combined with the indifferent return (37) for improved performance when placed on opposite sides of a patient. The excitation signal traverses the complex (tissue) load returning to the generator (16) via patch electrode and impedance is measured by observing the voltage drop across the sense connectors.

    • A medical catheter assembly (12) includes a deflectable catheter shaft (21), a distal tip with tip element and a mounting shaft (38), and a pull ring assembly (50), the distal tip (30) defining guide channels (41, 42) for pull wires (52, 53) of the pull ring assembly (50) such that the wires initially extend from the pull ring (51) toward the tip element and then loop back and through the catheter shaft (21) to handle actuator (120). Stress forces on the connection joints between the pull wires and the pull ring are dissipated, reducing the incidence of failure of the pull ring assembly. The catheter can be non-irrigated ablation catheter with a tip electrode, or an irrigated ablation catheter further including a fluid manifold (33).

    • Positioning of the distal end of a catheter at a desired location in an atrium of a patient's heart, can be determined or verified through measurement of blood gas values proximate the end. In one embodiment, an oximeter is used to monitor oxygen saturation to distinguish between de-oxygenated venous blood and well-oxygenated arterial blood. Optical signals may be transmitted to the distal end of the catheter and received therefrom via optical fibers. Specifically, a catheter (300) has a number of optical fiber ends (308,310,312) disposed at a distal end (304) thereof. A first fiber end (310) transmits the red and infrared optical signals. Fiber ends (308, 312) are used for detecting reflected optical signals which are then processed by a detector and oximeter instrument to provide oxygen saturation readings that indicating the position of the distal end of the catheter or successful penetration of the interatrial septum.
