摘要： A cardiac stimulation apparatus and multi-electrode pacing lead for prevention or treatment of atrial fibrillation. The multi-electrode lead will distribute electrodes throughout the right atrium to provide a choice of pacing sites at locations best suited to multi-focal control of the rapid atrial heartbeats that may precede atrial fibrillation. Cardiac electrical activity is sensed in four dimensions (three spatial dimensions, X, Y, and Z, and in time). The system triangulates the location of a given event in three spatial dimensions, and then paces the heart in areas that will disrupt the propagation of the arrhythmia. Termination involves delivering low voltage pulses to large areas of the heart, either simultaneously or in rapid sequence.

摘要： An apparatus (100) and method to determine the relative locations of a set of implantable electrodes (E1-Ed), preferably five or more, which can be positioned in a chamber of the heart (18) or within another organ or chamber. At least two electrodes are configured to be a known distance from each other. A signal generation apparatus provides a signal having a frequency through a set of pairs of electrodes. This signal is detected on all other electrodes. Additional pairs of signal-emitting electrodes are selected and measurements are made on all other electrodes until sufficient sets of data have been acquired specify a set of equations. Solution of the sets of equations by numerical methods provides the relative locations of the electrodes in a dielectric medium. Where the electrodes are implanted in an environment comprising dielectric media of differing characteristics, such as blood and myocardial tissue, the signal generation apparatus is capable of producing signals at multiple frequencies. Sets of equations are acquired for multiple frequencies. Solutions of the equations derived from each of the sets of equations are combined to eliminate the effect of a non-uniform dielectric medium. Alternatively, the effect of differing media can be eliminated by calculating position vectors for image charges or virtual electrodes. Location of a dielectric boundary, such as the myocardial wall, may be determined.

An implantable cardiac stimulation system with a multi-electrode lead having three or more selectable electrodes, together with apparatus for identifying an optimal subset of electrodes, apparatus for shaping a propagating wave front, and apparatus for modifying the intrinsic ventricular cardiac activation sequence, or generating simultaneous or near simultaneous pacing pulses to the septum or right ventricular outflow tract during ventricular systole in order to improve left ventricular cardiac efficiency and reduce mitral regurgitation in patients with dilated cardiomyopathy. A three dimensional map of electrode placement may be calculated. A sub set of the available electrodes in the right side of the heart is selected for stimulation such that septal motion during systole is reduced or the mitral valve area is stiffened to reduce mitral regurgitation.
