摘要： A medical device for viewing inside a body and for retrieving an object from a location within the body, the medical device comprising an endoscope (32) having a tube defining a working channel (36) and a basket (48) engageable with the object. In one embodiment of the invention, the basket (48) does not require a sheath and is disposed through the working channel such that the endoscope operates to contain or activate the sheathless basket. In another embodiment of the invention, a single push wire (66) with a collar (64) at its end is used in place of a sheath to control the opening and closing of the basket. In another embodiment of the invention, a tapered and funnel-shaped sheath (70) surrounding the basket is used to contain or activate the basket. The invention minimizes the basket crossing profile so as to improve fluid flow in the working channel.

摘要： A surgical device (50) is adapted to facilitate access through an incision (100) in a body wall (18) having an inner surface and an outer surface, and into a body cavity of a patinet. The device includes (55) and second (65) retention members adapted to be disposed in proximity to the outer surface and the inner surface of the body wall, respectively. A membrane (75) extending between the two retention members form a throat (90) which is adapted to extend through the incision (100) and form a first funnel extending from the first retention member (55) into the throat (90), and a second funnel extending from the second retention member (65) into the throat (90). The throat of the membrane has characteristics for forming an instrument seal in the presence of an instrument and a zero seal in the absence of an instrument. The first retention member (55) may include a ring with either a fixed or variable diameter.

摘要： A surgical access device (34) includes a single valve (35) that forms a seal with the body wall (21) and provides an access channel (32) into a body cavity (18). The valve (35) has properties for creating a zero seal in the absence of an instrument, an instrument seal in the presence of instruments, or a hand port to provide access into the cavity (18). The valve can include a gel, preferably an ultragel comprised of an elastomer and an oil providing elongation greater than 1000 percent and durometer less than 5 Shore A. A method for making the surgical access device includes the combining of a gelling agent with an oil, preferably by molding. A method for use includes steps for creating an opening with the instrument. The valve and method are particularly adapted for laparoscopic surgery wherein the abdominal cavity is insufflated with a gas.

摘要： A surgical retractor (10) is adapted for use in opening an incision (12) in a body wall (14) to facilitate access into a body cavity. The retractor includes an interior anchor (16) that is disposed interiorly of the body wall and an exterior tensioning device (56) that is disposed exteriorly of the body wall. At least one tension element (21) is coupled to the interior anchor and adapted to pass through the incision to engage the exterior tensioning device. Engaging elements (59, 62) are provided on the exterior tensioning device to engage the tension element and to maintain a tensile force on the tension element as it passes through the incision. Multiple concentric rings can also be used to maintain a tensile force on the tension element.

摘要： The invention relates to a hand access laparoscopy device having a gelcap, a retainer, a sleeve and a retention ring. The gelcap includes gel that is bonded to a cap. The cap includes an inner cylindrical Wall to which the gel in bonded, thereby providing a sealing area between the device and the wound in a body wall. By securing the gel to the inner cylindrical wall, the thickness of the gel and corresponding cap is minimized along with the overall footprint of the device. With the gel thickness reduced and able to be substantially flush, the "doming" phenomenon produced by insufflation of a patient's abdomen is reduced.
