    • A surgical suturing device is disclosed. The surgical suturing device has a forked guide tip having a plurality of legs, wherein each of the plurality of legs comprises a proximal end,an anatomical variation, and a distal end. A method of chord replacement for a heart valve is also disclosed. A suture is placed in a papillary muscle using a surgical suturing device having a forked guide tip and a papillary suture. A leaflet suture is placed in a leaflet. The papillary suture and the leaflet suture are loaded in a suture fastener from opposite directions in a coaxial fashion. The length of the papillary suture and/or the leaflet suture are adjusted relative to the suture fastener to achieve a desired replacement chord length. The suture fastener is attached to the papillary suture and the leaflet suture to lock the desired replacement chord length.

    • An instrument (10) is provided having a shaft (12) extending to a distal end (16). The distal end (16) has a distal tip member (40) having a cavity (42, 83), and a passageway (44, 80) extending to the cavity through which a first suture end (48A) of a loop of suture (48) extends from a tissue site to facilitate a user looping a second suture end (48B) around the first suture end (48A) between the distal tip member (40) and the tissue site to form a knot (48C). Advancing of the distal tip member (40) while drawing the first suture end (48A) through the passageway (44, 80) pushes the knot (48C) for placement at the tissue site. The distal tip member (79) of another embodiment provides an additional open-sided passageway (81) through which the second suture end (48B) is passed to assist in forming and pushing a knot at to the tissue site. A more secure knot (50) may be formed by a double loop (49A) and then a single loop (49B) by the second suture end (48B) around the first suture end (48A) and appropriate tensioning of the first and second suture ends when the knot is pushed to the tissue site. The distal end (16) further has two openings (46, 47) through which the two suture ends (48A, 4813) are passed from the placed knot (48C, 50), and a blade (38) in the distal end (16) is positionable to cut the two suture ends near the tissue site to trim the suture away from such knot.

    • A tissue manipulation apparatus (10) and method of use is provided to enable remote holding and multidirectional movement of tissue, such as the prostate gland to aid in its surgical removal during prostatectomy. The apparatus (10) has a housing (12) and a shaft (14) extending from the housing which curves to a distal end (16). The shaft (14) is insertable through a natural or surgical opening or channel in the body of a patient, such as the urethra (114) of a male patient to access the patient's prostate gland (116). Tissue engaging members (20) in the distal end (16) are actuated by a longitudinal drive mechanism through the shaft (14) from their retracted position at the distal end (16) to extend into the tissue. The distal end (16) is rotatable by a rotational drive mechanism extending through the shaft (14) to the distal end. The longitudinal and rotational drive mechanisms are independently operated at the housing (12). The tissue once held by the tissue engaging members (20) may be manipulated in its position under control of the operator of the apparatus (10), such as, raised or lowered by adjusting the tilt angle of the shaft (14), pulled or pushed by changing the extent of the shaft (14) passing through the opening or channel to access the tissue, and bi-directional rotated via the rotational drive mechanism.
