摘要： Endoscopic suturing devices for transgastric flexible endoscopy procedures are described. In a first aspect, the endoscopic suturing device includes a hollow helical needle carrying an elongated suture thread with a suture anchor on the distal end of the suture thread. The suture anchor has a low-profile insertion position and an expanded anchoring position. A suture lock is provided for attachment at the proximal end of the suture thread. In another aspect, the suture device includes a microcarabiner and suture lock. The suture device may take the form of a conventional suture thread or a zip-tie fastener. In an alternate configuration, the microcarabiner can be combined with a tissue anchor. One or more such tissue anchors can be used to form a running suture, purse string suture or other suture configurations. Methods of applying sutures using the endoscopic suturing devices are described.

摘要： Methods and devices are disclosed for manipulating the tongue (9). An implant (500) is positioned within at least a portion of the tongue (9) and may be secured to other surrounding structures such as the mandible (13) and/or hyoid bone (15). In general, the implant (500) is manipulated to displace at least a portion of the posterior tongue (410) in an anterior or lateral direction, or to alter the tissue tension or compliance of the tongue. Methods and devices for adjusting a glossopexy system are also disclosed. Furthermore, methods and devices for tensioning a glossopexy system are disclosed. A tether (2135) can include a tether protection element (2130) configured to prevent overextension of the tether (2135).

摘要： In one embodiment, a pressure sensing system is described which transmits data to a patient management system external to a patient. The pressure sensing system can rigidly couple to an implantable port or flexibly couple to an implantable port. In some embodiments, the pressure sensing system communicates with a hydraulic actuating system. In some embodiments, the pressure sensing system is implantable and comprises a circuit capable of wireless transmission through the skin of a patient to an external receiver which is part of a patient management system. A patient management system is described which receives up to date as well as historical data from the pressure sensing system and manages the these data in the context of a patient database. In some embodiments, an extragastric balloon is described in which the balloon is contoured to -..it ..i poi.jL?p o± the stomach nur not circumscribe the stomach. In some embodiments, electroactive polymers or nitinol structures are utilized to create restriction on the stomach in response to food boluses entering the stomach. In some embodiments, a nasogastric connector is described with two expandable structures translateable toward and away from one another so as to create pressure between two organ lumens when brought toward each other and fixed with respect to one another.

摘要： Methods and devices are disclosed for remodeling the tongue (9). One or more spaces or cavities (702) are formed in the tongue (9) using surgical or RF ablative techniques. The cavities (702) are closed or collapsed by inserting a tethered soft tissue anchor (500) into the tongue (9) and attaching the tethered portion of the soft tissue anchor (500) to a bony structure such as the mandible (13) or hyoid bone (15) in order to exert a collapsing force on the one or more spaces or cavities (702). The insertion pathway of the tethered soft tissue anchor (500) may pass adjacent to or even through one or more cavities (702). Also disclosed herein is a tongue remodeling system that includes means for creating a space in the tongue, and a tissue anchor (500) configured to close the space. The tissue anchor (500) may be tethered or expandable.
