摘要： An electrophysiology catheter (20) comprises a shaft (22) having a first bending stiffness and a deflectable tip (28) secured to the distal end (24) of the shaft with a second bending stiffness less than the first bending stiffness. At least one electrode (34, 36) is mounted to the tip for delivering current to or monitoring electrical activity of tissue. A manipulator wire (58) is coupled to the distal end of the deflectable tip, whereby the deflectable tip may be deflected by axial force applied to the manipulator wire. A stiffener member (66) is axially slidable relative to the tip so as to adjust the tip curvature without removing the catheter from the body. The catheter may further include a core wire (72) configured to rotate the deflectable tip about a longitudinal axis (2) without rotating the proximal end (26) of the catheter shaft, wherein the distal end of the deflectable tip remains in a substantially constant axial position, preferably in a plane perpendicular to the longitudinal axis.

摘要： An embodiment of the present invention is directed to methods and devices for treating diseases and conditions that change the special relationship between vertebral bodies and intervertebral disks. A method for performing a transpedicular discectomy procedure may include creating a transpedicular channel to a first vertebral body through a first pedicle of a first vertebra; inserting a flexible drill through the transpedicular channel causing the flexible drill to make an approximately 90 degree angle, the flexible drill creating a channel through the first vertebral body into an intervertebral disk; and removing a portion of the intervertebral disk with a laser device. A laser catheter device for use in ablation and removal of intervertebral disk material in a percuntaneous transpedicular approach may include an elongated tube comprising a first lumen and a second lumen, the first lumen comprising a fiber optics bundle and the second lumen for evacuation of ablated material; and a Holmium-YAG infrared laser or a laser diode for generating laser energy to the distal end through the elongated tube.

The catheter comprises an elongated, flexible tubular catheter body having proximal and distal ends and a lumen extending therethrough and a control handle at the proximal end of the catheter body. The control handle comprises a handle housing having a generally hollow interior and fixedly attached, either directly or indirectly to the catheter body; a piston slidably mounted in the handle housing and having proximal and distal ends; and a pulley fixedly attached, either directly or indirectly to the handle housing at a location proximal to the proximal end of the piston. A first puller wire is provided having a distal end fixedly attached in the distal end of the catheter body and a proximal end anchored to the piston. A second puller wire is provided having a distal end fixedly attached in the distal end of the catheter body. The second puller wire extends through the catheter body, into the handle, and around the pulley, and the proximal end of the second puller wire is anchored to the piston.

摘要： A bidirectional steerable catheter is provided that can be deflected to form two different curves, i.e., two curves each having a different radius of curvature. The catheter comprises an elongated, flexible tubular catheter body having proximal and distal ends and a lumen extending therethrough. A tip section is provided at the distal end of the catheter body. The tip section comprises a flexible plastic tubing having at least first and second off-axis lumens extending therethrough. A control handle is attached to the proximal end of the catheter body. A first puller wire extends through the lumen of the catheter body and the first off-axis lumen of the tip section. The first puller wire has a distal end anchored in the tip section and a proximal end anchored to the control handle. A second puller wire extends through the lumen of the catheter body and the second off-axis lumen of the tip section. The second puller wire has a distal end anchored in the tip section and a proximal end anchored to the control handle. A first compression coil extends through the lumen of the catheter body in surrounding relation to the first puller wire. The first compression coil has a distal end anchored in the catheter body or in the first off-axis lumen of the tip section at a first anchor position. A second compression coil extends through the lumen of the catheter body in surrounding relation to the second puller wire and into the second off-axis lumen. The second compression coil has a distal end anchored in the second off-axis lumen of the tip section at a second anchor position that is distal to the first anchor position. By this design, the radius of curvature of each curve achieved by deflection of the tip section is controlled by the anchor position of the compression coil surrounding the appropriate puller wire.

摘要： A system for forming a purse string suture, including a suture applicator having a proximal handle having a suture spool mounted thereon or in an internal chamber. A length of suture is partly wound on the spool, and a hollow suture needle extends from a distal end of the handle and contains a free end of the length of suture. The suture needle projects distally from the handle in a helical shape and terminates in a sharp distal tip with an opening in one side close to the distal tip. The free end of the length of suture may extend out of the distal tip and back into the hollow suture needle through the opening. The suture needle may have a deflection segment adjacent the distal tip that is more flexible than the rest of the helical distal portion of the suture needle. A linear hollow pivot shaft extends from a distal end of the handle substantially along an axis of the helical distal portion of the suture needle. The pivot shaft has a blunt tip with a substantially larger radial profile than the shaft to prevent puncturing tissue.

摘要： A dual curve ablation catheter (2), especially suited for treating atrial flutter, includes a shaft (4) with a deflectable tip (20) at the distal end (6) and a handle (10) at the proximal end (8). The tip includes a highly flexible distal segment (30), a relatively stiff intermediate segment (28) and a flexible proximal segment (26) so that pulling on a manipulator wire (16) attached to the distal segment causes the distal segment to curve and engage, for example, an isthmus of tissue (106) adjacent the tricuspid valve (104) and the inferior vena cava (98) and causes the proximal segment to curve and press against the wall (110) of the inferior vena cava so to stabilize the catheter. Ablation energy can be supplied through the ablation electrodes (48, 68) simultaneously or one at a time to ablate tissue at the isthmus without the need for moving the catheter.
