    • Guidewire-deliverable devices and methods of using the devices for separating adjacent layers of tissue and for delivering a surgical prosthesis to an operating site. A guidewire-deliverable cannula provides an elongate window for accessing the operating site with a tissue-separating device. The tissue-separating device may include one of several types of an inflatable balloon or a tissue-dissecting member. The tissue-separating balloon may simultaneously deliver the prosthesis during tissue separation and may further be detached to remain with the prosthesis over the operating site. Alternatively, the prosthesis may be separately provided after the adjacent layers of tissue have been separated.

    • Described is a tissue resection system which includes a guide element; a resection head; a positioning mechanism and an actuating mechanism. The resection head includes a guide channel slidably receiving the guide element and an endoscope channel sized to slidably receive an endoscope therein, so that, when the resection head is slid along the guide element, the endoscope and the resection head follow a path defined by the guide element. The positioning mechanism moves the resection head relative to a distal end of the endoscope between a first position in which the distal end of the endoscope extends distally out of the endoscope channel and a second position in which the distal end of the endoscope is received within the endoscope channel. The actuating mechanism moving a portion of an outer wall of the resection head to open and close a resection chamber defined by the outer wall.

    • An applicator is provided for applying a sheet of surgical material, e.g., an adhesion barrier or other surgical fabric, to internal body tissue. The applicator includes an expandable operating tip which is insertable into an endoscopic tube through an incision in a body to enable a surgeon to apply the adhesion barrier or other surgical fabric to tissue inside the body. A preferred embodiment of the applicator includes a set of telescoping tubes comprising an outer delivery tube, a intermediate deployment tube, and a inner irrigation tube. An expandable spreader tip is mounted at the distal end of the irrigation tube and connected to the distal end of the deployment tube. By advancing the deployment tube and irrigation tube relative to the delivery tube, the spreader tip is exposed at the distal end of the delivery tube. The spreader tip is expanded by movement of the deployment tube relative to the irrigation tube to spread the adhesion barrier over the tissue. A nozzle is provided at the distal end of the irrigation tube for applying a fluid, e.g., a saline solution, to the adhesion barrier.

    • A tissue manipulator is provided which is insertable through an endoscopic tube to enable a surgeon to manipulate tissue inside a body cavity. The endoscopic tissue manipulator includes an expandable tissue manipulating frame adapted to expand transversely through a longitudinal slot in a support tube to provide a spatula-shaped platform which can be manipulated by rotation of the support tube about its longitudinal axis to engage and displace large body organs away from a desired surgical site in a body cavity. The frame consists of a plurality of flexible frame members or wires adapted to flex laterally outward through the longitudinal slot in the support tube into a bow-shaped configuration overlapping the other frame members or wires of the spatula-shaped platform. The instrument can be provided with a single tissue manipulating frame or a pair of frames which are expandable through separate longitudinal slots in the support tube to provide a pair of spatula-shaped platforms to engage the tissue. The pair of frames of the dual frame embodiment may be actuated together or independently.

The shell of fine grains formed on the cast bar during the first stage of compressive deformation permits substantial reduction of the cross-sectional area of the cast bar during the second stage of rolling deformation without the cast bar cracking, even when the cast bar has a high impurity content or is otherwise susceptible to intergranular rupturing.
