    • A method and programmer for programming a neurostimulation device are provided. The method comprises varying a first stimulation parameter under user control, automatically varying a second stimulation parameter, generating a plurality of stimulation parameter sets from the varied first and second stimulation parameters, outputting a pulsed electrical waveform from the neurostimulation device between a plurality of electrodes in accordance with the stimulation parameter sets, such that neural tissue is stimulated without undergoing neurological accommodation that would otherwise occur if the second stimulation parameter were not varied, and programming the neurostimulation device with a new set of stimulation parameters based on a result of the neural tissue stimulation The programmer comprises a user interface capable of receiving an input from a user, a processor configured for performing the previous steps, and output circuitry configured for transmitting the stimulation parameter sets and the new stimulation parameter set to the neurostimulation device.

The method comprises placing an array of electrodes in proximity to the tissue, conveying electrical current between the electrodes of the array to stimulate a tissue site, incrementally shifting the electrical current from at least one cathode to at least another cathode over a first range of fractionalized current values, and incrementally shifting the electrical current from at least one anode to at least another anode over a second range of fractionalized current values.     • A method and external control device for providing therapy to a patient using first and second electrodes implanted within the patient is provided. A train of electrical multi-phasic pulses is generated. A first electrical current is sourced from the second electrode and at least a portion of the first electrical current is sunk to the first electrode during a stimulation phase of each multi-phasic pulse, thereby therapeutically stimulating a first tissue region adjacent the first electrode.     • A method for sensing cardiac activity in an atrium of a patient's heart includes delivering a pulse to the atrium using an electrode configuration that includes at least a cathode electrode; sensing cardiac activity in the atrium using a unipolar electrode configuration to provide a sensed signal wherein the unipolar electrode configuration does not include the cathode electrode; determining the duration during which the voltage of the sensed signal falls below a threshold voltage; and comparing the determined duration to a parameter to determine whether the pulse caused an atrial evoked response.
