    • Suture clips and suture clip loading, delivery, locking and severing devices in both catheter and endoscopic embodiments are disclosed that permit the delivery and application of suture clips directly in contact with or in close approximation to sutured tissue. The catheter or endoscopic versions incorporate a collet cage with a plurality of flexible collet fingers that captivate a suture clip ring and a suture clip plug for assembly with a suture. An outer sliding sleeve constrains the collet fingers when distally advanced to minimize potential trauma during delivery and assembly of the suture clip in close proximity to the stitched tissue. Proximal retraction of the sliding sleeve coupled with the distal advancement of a pusher against cam surfaces on the inner walls of the collet fingers causes the collet fingers to open. The catheter is designed to hold and assemble the suture clip in a first position and release the suture clip and sever the suture ends proximal to the suture clip in a second position. Single pusher two-step embodiments and double pusher three-step embodiments are disclosed. Suture clip designs with plugs having heads for use with the suture clip delivery catheter are also disclosed. A suture clip loading device for loading suture clip components into the delivery system as well as a threader for threading sutures into the suture clip systems are also disclosed. A method of loading, delivering and deploying suture clips is further disclosed.

    • A combination tissue apposition and suture capturing device (100) for performing endoscopic procedures typically in the gastro-esophageal tract. The device (100) is particularly adapted for forming multiple plications used in a gastroplasty procedure devised to cure or ameliorate gastro-esophageal reflux disease. The device includes a tissue sewing capsule (102) attached to the distal end of an endoscope having a needle (120) that is deposited in a capsule (102) distal tip cavity following the suturing of a tissue fold and retrieved to enable the suturing of a subsequent tissue fold without the need for multiple intubations. A suture clip delivery device (200) is also disclosed that is adapted to fit within the capsule (102) to enable suture (118) capture without the need for multiple intubations. The combination device eliminates the need for an overtube and maximizes the speed efficiency of the gastroplasty procedure. A method for using the combination device is also disclosed.

    • The present invention provides a catheter positioning system which serves to control and stabilize a distal end of a catheter at a treatment site within a patient so that a medical procedure can be performed with accuracy. Generally, the positioning system operates by providing a deformable mechanical members at the distal end of the catheter which can be operated from the proximal end of the catheter to extend radially outward to engage surrounding tissue adjacent to treatment site. In one embodiment of the invention a flexible superstructure comprising the plurality of flexible veins extending longitudinally along the distal end of the catheter can be deformed to bow radially outward to engage surrounding tissue. The distal tip of the catheter joined to one of the veins was correspondingly displaced or rotated angularly as the veins bow outward. In another embodiment radially projecting fingers are joined to the distal end of the catheter, which remain retracted during navigation of the catheter to the treatment site then are extended outward to penetrate the tissue and secure the catheter at the treatment site upon being actuated from the proximal end of the catheter by a physician. Methods of positioning a catheter are also disclosed. The inventive device and method are particularly useful in catheter based procedures carried out in large body lumens or in cavities of body organs. In particular, the invention may be useful in delivering implants percutaneously through the left ventricle into the myocardium of the heart.

    • A method and apparatus are provided for releasably storing the additional exchange length of a medical guidewire within a housing, in a compacted configuration until it is needed by the physician to perform a catheter exchange. This guidewire has a distal portion which is similar to a traditional guidewire in length and construction and is used throughout the catheterization procedure much like a conventional guidewire. A proximal portion of the guidewire provides added length to effect a catheter exchange in the manner of a conventional exchange wire but may be tightly compacted and stored out of the way within a storage receptacle until the moment that the added length is needed for an exchange. The storage receptacle enables the physician to utilize the distal portion of the guidewire wire much like a conventional guidewire, then release the proximal portion quickly to effect a catheter exchange. The readily available additional length of wire offers the physician all the advantages of a traditional exchange wire while avoiding the handling problems associated with an uncontained exchange length that is unneeded during much of the procedure. The guidewire wire containment system of the present invention simplifies catheterization procedures by enabling the physician to maintain wire position during an exchange while keeping the exchange length of wire conveniently contained until it is needed to complete the procedure.

    • A guide wire system for use in catheter exchanges avoids the need for a separate exchange wire. Instead of the conventional practice in which the initial guide wire is removed and replaced with a longer exchange wire, a guide wire extension is attached to the proximal end of the initial guide wire thereby increasing its effective length so that it may be used to perform a catheter exchange. The initial guide wire remains in place in the patient's vasculature. The proximal end of the guide wire and the distal end of the exchange wire are formed to define a connection which may be crimped to effect the connection between the two wires. A crimping tool is provided to hold the mating ends of the guide wire and extension wire together while effecting the crimp.
