The second connection terminal is formed on the other side of the top surface of the base plate and includes an output terminal (132) on the lateral surface. The first optical fiber is formed on the top surface of the base plate and connected to the input terminal. The optical coupler splits lights emitted by the first optical fiber to the second and third optical fibers. An end portion of the second optical fiber is connected to the output terminal, and the second optical fiber includes a first Bragg grating (161).
