    • The present invention provides an intravascular therapeutic operating device which is used to cure blood vessels by removing chronic total occlusion (CTO) lesion within blood vessels. The intravascular therapeutic operating device of the present invention includes: a driving source which is provided to a case; a power transfer member which receives driving power of the driving source and rotates; a punching member which is combined to a front-end unit of the power transfer member; a punching member moving unit which is combined to the case and moving the punching member in the longitudinal direction; a guide pipe which is combined to the case and is arranged on the periphery of the power transfer member and punching member; a pulling line of which one side is combined to a front-end of the guide pipe; a guide pipe direction conversion unit to which the other end of the pulling line is combined for pulling the pulling line or the releasing the pulling condition of the pulling line; a blood vessel damage prevention pipe which is arranged on the periphery of the guide pipe; a support unit which is combined to the case and fixating one end of the blood vessel damage prevention pipe; and a blood vessel damage prevention pipe moving unit which is combined to the case and support unit for moving the blood vessel damage prevention pipe to a longitudinal direction. In the guide pipe, a plurality of direction conversion grooves with predetermined intervals is provided on the outer surface of the front-end unit.
