摘要： A device for incising a stenosis in the aortic valve of a patient includes an elongated balloon catheter, with at least one straight blade mounted on the balloon. Specifically, the blade is coplanar with the axis of the catheter, and the proximal end of the blade is located adjacent the proximal end of the balloon. In operation, the balloon/blade combination is advanced into the vasculature and positioned distal to the stenosis to be incised. The balloon is then inflated. With this inflation, the blade is inclined relative to the axis of the catheter with an increasing distance between the blade and the axis in a distal direction. The device is then retracted, proximally, to incise the stenosis.

摘要： Cross stream thrombectomy catheter and system for fragmentation and removal of thrombus or other material from blood vessels or other body cavities. High velocity saline jets emitted from a toroidal loop jet emanator or other jet emanator in a catheter distal end entrain fluid through inflow orifices, and with flow resistances create a back-pressure which drives cross stream streams through outflow orifices in a radial direction and thence radially and circumferentially to apply normal and drag forces on thrombotic deposits or lesions in the blood vessel or other body cavity, thereby breaking apart and transporting thrombus particles to be entrained through the inflow orifices, whereupon the high velocity jets macerate the thrombus particles which then transit an exhaust lumen or recirculate again via the outflow orifices.

摘要： A hollow microneedle with a substantially sharp edge is provided that includes at least one longitudinal blade at the top surface or tip of the microneedle to aid in penetration of the stratum corneum of skin. In a preferred embodiment, there are two such longitudinal blades that are constructed on opposite surfaces at approximately a 180° angle along the cylindrical side wall of the microneedle. Each edged blade has a cross-section that, when viewed from above the microneedle top, has an isosceles triangle profile. The blade's edge can run the entire length of the microneedle from its very top surface to its bottom surface where it is mounted onto the substrate, or the edge can be discontinued partway down the length of the microneedle. A star-shaped solid microneedle also is provided having at least one blade with a relatively sharp edge to assist in penetrating the stratum corneum of skin. In a preferred embodiment, a three pointed star-shape is used, in which each blade has an isosceles triangular cross-section when viewed from the top of the microneedle. The base of each of the isosceles triangles meets at a center of the microneedle to form the star-shaped structure. At least one hole through the substrate is located near the side surfaces of the pairs of blades of the solid microneedle.

摘要： An ultrasonic catheter drug delivery device comprises an ultrasound transducer to produce ultrasonic waves, which transducer is mechanically attached to a catheter body or chamber. The ultrasonic transducer has a distal tip with a distal radiation surface, and when a therapeutic agent from a fluid source is directed to the catheter body or chamber, the radiation surface creates ultrasonic pressure and delivers liquid and simultaneously ultrasonic energy to a patient's vascularity or a selected body lumen. The method applies therapeutic agent and ultrasonic waves to the vascular area, lung or any body lumen without requiring direct contact between ultrasound transducer and body, dissolves blood clots, and stimulates tissue cells.

摘要： An ophthalmological device for cutting into a nucleus of a cataract comprises a phaco tip including a body having at a distal end thereof a solid blade with a face edge adapted to be placed in contact with the nucleus. There is no aspiration port in this face edge. An aspiration port in the body is displaced rearward from the face edge by a distance which is greater than the length of the blade. A sleeve fits over the body and has a forward section that terminates in an opening in advance of the aspiration port. The device is used alternately with a standard phaco tip to remove cataracts. A surgical instrument comprises a handpiece including phaco tip having a face edge having aspiration port therein that is adapted to be placed in communication with a source of aspiration. A control unit regulates the application of aspiration to the aspiration port, enabling aspiration at the aspiration port to be discontinued on command.

摘要： A therapeutic ultrasound instrument is described for cutting, dissecting, or cauterizing tissue. Ultrasonic vibrations, when transmitted to organic tissue at suitable energy levels and using a suitable end-effector, may be used for the safe and effective treatment of many medical conditions. An ultrasonic surgical instrument is described which incorporates an articulating shearing end-effector. The instrument comprises an ultrasonic transducer, an ultrasonically activatable end-effector, and a substantially solid ultrasonic waveguide that connects the ultrasonic transducer to the end-effector. The waveguide comprises a transmission section extending from the transducer to a fixed node, and an articulation section extending from the fixed node to a pivoting node. The end-effector includes a bifurcated waveguide segment. A handle is adapted to hold the transducer. An outer sheath extends from the handle to the end-effector and surrounds the waveguide. An actuation trigger is rotatably positioned on the handle, and an actuation arm extends from a distal end of the actuation trigger to the pivoting node. Such instruments are particularly suited for use in minimally invasive procedures, such as endoscopic or laparoscopic procedures.
