    • A needle assembly having a single-handedly activated needle barrier includes a needle cannula having a lumen therethrough. A needle hub having an interior cavity terminating at an open proximal end of the hub is connected to the needle cannula so that the lumen is in fluid communication with the interior cavity. A guide element having a retaining groove therethrough is connected to the needle hub. An elongate barrier arm having a proximal end and a distal end includes a barrier element at its distal end. The barrier element includes a proximal end and a needle passageway therethrough. The barrier arm is positioned within the groove of the guide element and the needle cannula is positioned at least partially within the barrier element. The barrier arm is movable from at least a first retracted position wherein the distal end of the cannula passes through the barrier element so that the distal end of the needle cannula is exposed, to a second extended position wherein the barrier element surrounds the distal end of the needle cannula to help prevent accidental contact with the distal end of the needle cannula. Locking structure to prevent movement of the barrier arm from the second extended position is activated by movement of the barrier arm into the second extended position.

    • A safety trocar instrument, for piercing the wall of an anatomical cavity to provide communication with the inside of the cavity, includes a tubular cannula and an elongate trocar having a sharp piercing point. The trocar is mounted for axial reciprocal movement within the cannula between a withdrawn rest position in which the point is received within and shielded by the distal end of the cannula and a projecting position in which said point is exposed beyond the distal end of the cannula. A retraction spring biases the trocar to its withdrawn position. A latch latches the trocar in the projecting position in opposition to the retraction spring, and a trip member can trip the latch to unlatch the trocar and thereby permit the trocar to be moved to its withdrawn rest position by the retraction spring. A mechanical trigger in the form of a trigger sleeve is mounted within the instrument. Its distal end is locatable between the extreme of said trocar point and the distal end of the cannula to sense counterforce exerted by the cavity wall after it has begun to be pierced by the trocar point and to sense relief of such counterforce when that distal end has cleared the cavity wall. The trigger sleeve is mechanically coupleable to the trip member to cause it to trip the latch in response to sensing relief of such counterforce.

    • A linear surgical stapling instrument includes a magazine for holding a plurality of undeformed surgical staples, and an upper jaw carrying the magazine and having opposed distal and proximal ends. An anvil is formed with staple-deforming pockets, and a lower jaw carries the anvil and has opposing distal and proximal ends respectively configured to mate with the distal and proximal ends of the upper jaw thereby to place the magazine and the anvil in confronting relation. A staple driver is carried in the upper jaw for driving staples held therein toward the anvil. Further, the upper and lower jaws are configured such that, when their respective distal and proximal ends are mated, a gap exists between the jaws in an intermediate region between their respective distal and proximal ends. Finally, a pivotably mounted clamp bar joins the upper and lower jaws and when actuated compresses them together in the intermediate region thereby to narrow the gap and to increase the clamping pressure between the magazine and the anvil. The instrument also includes a blocking system that prevents the jaws from being remated, once having been mated and having had staples in the magazine driven toward the anvil. Still further, the instrument includes a coupling device for coupling the jaws and clamp bar together when the staple driver is actuated.

    • A system for performing an end-to-side vascular anastomosis. including an anastomosis device, an application instrument and methods for performing a vascular anastomosis. The system is applicable for performing an anastomosis between a vascular graft and the ascending aorta in coronary artery bypass surgery, particularly in port-access CABG surgery. A first aspect of the invention includes a vascular anastomosis staple. A first configuration has two parts: an anchor member, forming the attachment with the target vessel wall and a coupling member, forming the attachment with the bypass graft vessel. The anastomosis is completed by inserting the coupling member, with the graft vessel attached, into the anchor member. A second configuration combines the functions of the anchor member and the coupling member into a one-piece anastomosis staple. A second aspect of the invention includes an anastomotic fitting, having an inner flange over which the graft vessel is everted and an outer flange which contacts the exterior surface of the target vessel. A tailored amount of compression applied by the inner and outer flanges grips the target vessel wall and creates a leak-proof seal between the graft vessel and the target vessel. A third aspect of the invention has a flange to which the graft vessel attaches, by everting the graft vessel over the flange, and a plurality of staple-like members which attach the flange and the everted end of the graft vessel to the wall of the target vessel to form the anastomosis
