    • A system for performing an end-to-side vascular anastomosis, including an anastomosis device, an application instrument and methods for performing a vascular anastomosis. The system is applicable for performing an anastomosis between a vascular graft and the ascending aorta in coronary artery bypass surgery, particularly in port-access CABG surgery. A first aspect of the invention includes a vascular anastomosis staple. A first configuration has two parts: an anchor member, forming the attachment with the target vessel wall and a coupling member, forming the attachment with the bypass graft vessel. The anastomosis is completed by inserting the coupling member, with the graft vessel attached, into the anchor member. A second configuration combines the functions of the anchor member and the coupling member into a one-piece anastomosis staple. A second aspect of the invention includes an anastomotic fitting, having an inner flange over which the graft vessel is everted and an outer flange which contacts the exterior surface of the target vessel. A tailored amount of compression applied by the inner and outer flanges grips the target vessel wall and creates a leak-proof seal between the graft vessel and the target vessel. A third aspect of the invention has a flange to which the graft vessel attaches, by everting the graft vessel over the flange, and a plurality of staple-like members which attach the flange and the everted end of the graft vessel to the wall of the target vessel to form the anastomosis.

    • A trocar assembly consisting of a trocar tube subassembly and a separate interengageable trocar body subassembly is provided. The trocar body subassembly includes an elongated obturator having a sharp pointed tip which is normally housed within an elongated tube of the trocar tube subassembly. The obturator is extendable from a retracted position within the tube wherein the tip is recessed from the tube end to an operative position forward of said tube end and outside the tube. By manually advancing the trocar assembly against a body wall, the resisting force locks the obturator in the operative position. Relaxation of the force incident to passing through the body wall causes the obturator to be automatically unlocked and retracted to its initial position with the tip positioned protectively within the tube, thereby protecting body tissues and organs from being damaged by the tip when it enters a body cavity beyond the wall.

    • A trocar assembly comprising an elongate trocar obturator having a piercing tip at its front end, an elongate trocar tube in which the obturator is housed, and a tubular protective shield mounted concentrically around the obturator between a normally extended position in which the obturator tip is covered and a retracted position in which the obturator tip is exposed. Further included is a spring acting on the protective shield, whereby the shield is forced to the retracted position to expose the piercing tip when the trocar is being inserted through the wall of a body cavity and is biased by the spring to the extended position to shield the piercing tip once the trocar has pierced the wall. A protrusion extends radially from the shield. A leaf spring element has one end anchored relative to the obturator and a second end biased to contact the protrusion in a manner preventing movement of the shield from the extended position toward the retracted position when the element is in contact with the protrusion. A trigger, coupled to the leaf spring member is shiftable between a lock position and a release position. When the trigger is in the lock position, the shield can not be moved from the extended position. When it is in the release position, the shield can travel to the retracted position. When it moves to the retracted position, it disengages the trigger so that when the shield returns to the extended position, it locks in place, even through the trigger is in the release position.
