    • SE MUESTRAN DISPOSITIVOS Y PROCEDIMIENTOS PARA REALIZAR OPERACIONES QUIRURGICAS PERCUTANEAS BAJO VISUALIZACION DIRECTA Y A TRAVES DE UNA UNICA CANULA. UN DISPOSITIVO (10) INCLUYE UNA CANULA ALARGADA (20) QUE TIENE UN PRIMER DIAMETRO INTERIOR (DI ) Y UN DIAMETRO EXTERIOR (D O ) DIMENSIONADOS PARA INTRODUCCION PERCUTANEA. LA CANULA (20) DEFINE UN CANAL DE TRABAJO (25) ENTRE SUS EXTREMOS (21,22) QUE TIENE UN SEGUNDO DIAMETRO (D 2 ) IGUAL AL DIAMETRO (D 1 ) DE LA CANULA, DIM ENSIONADO PARA RECIBIR UNA HERRAMIENTA A TRAVES DEL MISMO. EN LA CANULA (20) SE PUEDE INTRODUCIR UN ELEMENTO ALARGADO PARA VISIONADO (50), ADYACENTE AL CANAL DE TRABAJO (25), PREFERENTEMENTE POR UN ACCESORIO (30, 170). EL ACCESORIO (30, 170) INCLUYE UN ALOJAMIENTO/CUERPO (31, 171) QUE PUEDE ACOPLARSE AL EXTREMO PROXIMAL (22) DE LA CANULA (20). EL ALOJAMIENTO (31) DEFINE UN DIAMETRO INTERIOR PARA LA OPTICA (60, 180) QUE SOPORTA UN ELEMENTO DE OBSERVACION (50) PARA TRASLACION Y ROTACION RESPECTO A LA CANULA (20) DE MANERA QUEEL EJE LONGITUDINAL DEL ESPACIO INTERIOR DE LA OPTICA (60, 180) SE DESPLACE PARALELAMENTE A, Y GIRE ALREDEDOR DE, EL EJE LONGITUDINAL DE TRABAJO (25). SE PROPORCIONAN TAMBIEN PROCEDIMIENTOS PARA REALIZAR OPERACIONES QUIRURGICAS POR VIA PERCUTANEA, CON VISUALIZACION DIRECTA Y SIN EXIGIR UN ESPACIO DE TRABAJO MANTENIDO POR FLUIDO.

    • Devices and methods for performing percutaneous surgery under direct visualization through a single cannula are shown. A device includes an elongated cannula (20) sized for percutaneous introduction that defines a working channel (21) between its ends. The working channel (21) is sized to receive one or more surgical tools therethrough. A clamp assembly (350, 400) is engaged to the outer surface of the cannula (20). The clamp assembly (350, 400) includes a viewing element receiving portion (390) and a viewing element (310) includes a clamp assembly engaging portion for removably engaging the viewing element (310) to the clamp assembly (350, 400). In one embodiment, the receiving portion (390) defines a receptacle for receiving a dovetail (330) included on the engaging portion of the viewing element (310). The receiving portion (390) may also include a pivotable clip (340) for removably engaging a protuberance (335) of the engaging portion. The clamp (350, 400) includes a mechanism that allows translation, rotation, and removal of the clamp assembly (350, 400) and the viewing element (310) with respect to the cannula (20). The cannula (20) may also be engaged to a flexible arm (160) that is securely mounted in order to stabilize the position of the cannula (20) at the surgical site.
