    • A computerized system and method for generating CMS-compliant health services delivery (HSD) tables. A database stores health care service provider data. A remote computer user interacts with a server that provides access to the health care service provider data. A user can view health care service provider data that meets selected search parameters. The data is displayed in an interactive worksheet. The user may further cause amendments to be made to the data stored in the database. The user may issue a request to generate an HSD table in “final file format” which conforms to CMS requirements. The server checks for errors in the file and notifies the user of errors that are identified. The server optionally checks for adequacy of the network associated with a plurality of health care service providers. The user may approve the final files for filing with CMS.

    • A method for supporting heavy equipment at a site is provided. The method includes constructing a plurality of pre-formed reinforced concrete blocks. Each of the concrete blocks is constructed by: providing a concrete form having internal dimensions of at least 1½ feet by 3 feet by 3 feet, inserting a plurality of reinforcing members within the form, pouring concrete into the form over the reinforcing members, allowing the concrete to cure to form a concrete block, and removing the concrete block from the form. The method further includes excavating the site to form an excavated area and placing the plurality of pre-formed reinforced concrete blocks on the excavated area. The present invention also provides a concrete pad which can be formed by the above-described method.

    • A method for generating a personal health record for a patient includes: providing a plurality of medical claim objects that are stored in a first format, the medical claim objects including medical claim codes indicative of medical claim events; and translating the medical claim objects from the first format into a second format to generate personal health record data. The personal health record can also include patient or practitioner entered data. The personal health record is portable and may be owned by the patient. Access to the personal health record can be given by a patient to a practitioner to the extent desired. The portable personal health record is stored in standard codes so that medical information, advertisements, queries, and the like can be readily translated to various reader levels to facilitate clear communication at the reader's education level and language.
