    • The old adage "Necessity is the mother of invention" holds true but it is in the mind of the beholder. SSM including NSM is a challenging, technically demanding and time consuming procedure. It represents a challenge to the surgeon in regard to the ability to perform a complete and through glandular mastectomy enbloc with axillary nodal dissection from within the narrow, small confines of the periareolar hole. An ideal SSM procedure produces a uniform thickness skin flap with no much inclusion of mammary tissue within, while simultaneously not risking flap viability. Energy-based device technology is a marvelous recent addition to the armamentarium of open and endoscopic surgery that has greatly facilitated hemostatic tissue dissection and vessel sealing. So it was plausible to think of an Energy-Based Device specified for SSM procedure to make it an easy fast task with enhanced ergonomicity. The new invented EVSD has a long curved half-cylinder shaped jaws that on closure form a Hegar`s dilator like cylindrical rod with a blunt tipped conical end. On creating skin flaps of SSM, such unique jaw configuration that works as a Hegar`s dilator allows for the formation of long juxta-tunnels through the subcutaneous fat space along the plane of the SL of SF, then grasping tissue bridges and blood vessels between the created adjacent tunnels. The combined retractable monopolar L-hook is used in an alternate way with the Hegar`s dilator shaped jaw for incision and dissection of pectoral fascia, interpectoral fascia, clavi-pectoral fascia, and subscapular fascia with sweeping off the axillary contents in bloc, so the whole procedure of SSM is done with an only one device in a quick and more ergonomic manner, obviating the need for instrument exchange and very much applications needed when using other conventional energy based devices with small unsuitable jaw configuration, in addition to fulfilling the orthodox aesthetic and oncologic safety standards.
