摘要： A tissue closure assembly and a closure delivery device are provided for closing incisions in a target tissue, such as an artery, vein, fascia or hollow organ. The closure delivery device has a closure deployment mechanism within an elongated cylindrical body with an expanding dilator tip and a control handle with an actuation mechanism for sequentially actuating the expanding dilator tip and the closure deployment mechanism. A lumen locator and anchor mechanism has a guidewire and back bleed lumen, an expandable anchor and, optionally, an arteriotomy cutting blade. Various embodiments of tissue closure assemblies are provided having a first and second tissue closure component for attaching to the target tissue on either side of an intended arteriotomy site and means for approximating the first and second tissue closure component and locking them together to seal the arteriotomy after performing a transluminal procedure through the arteriotomy.

摘要： A vessel access and closure device places a running suture in the wall of a blood vessel using a rotating helical suture needle that carries the suture along a helical path passing through the vessel wall, then reverses rotation to release a suture anchor attached to the distal end of the suture. A loose helical coil of suture is left behind as the helical suture needle withdraws. The device is withdrawn and replaced with a vessel dilator and an introducer sheath that opens up a larger access opening into the blood vessel and creates a pathway that passes though the helical coil of suture for introducing an interventional device into the blood vessel. After the interventional procedure is completed, the interventional device and the introducer sheath are withdrawn and the running suture is tightened and secured with a suture lock to close the access opening into the vessel.

摘要： Devices and methods for creating and closing controlled, shaped openings in tissue, such as blood vessel walls or fascia layers, utilize an implantable access and closure device, or access port, with an aperture that demarcates a desired location for the opening in the tissue. Incision means for creating an opening through the tissue may include a crescent or arc-shaped heating element; a cutting wire; a water jet; or a cutting edge that is integrated into the access port. A self-sealing incision is also described. Closure means to seal the arteriotomy or other opening and provide hemostasis may include a net, patch, ring or wire that is deployed externally to the vessel, covering the arteriotomy site. Another closure means comprises a stent, or tube-like closure mechanism that is inserted through the arteriotomy site and deployed within the vessel. A side opening allows a procedure sheath to enter the blood vessel. After the procedure, the closure mechanism is rotated within the vessel so that the side opening is no longer aligned with the arteriotomy.

摘要： Methods and apparatus for providing transapical access to a heart chamber for performing an intra cardiac procedure are described. The apparatus include a helical needle driver, a dilator, a straight access needle, and optionally a guidewire. After entering the heart chamber with the straight access needle, the helical needle driver is used to place a helical suture within the myocardium. After removing the needle driver, the dilator is advanced through the pre-placed helical suture, dilating both a passage and the circumscribing suture. After performing procedure, the pre-placed suture may be closed by proximally retracting an external end of the suture.

摘要： A tissue closure assembly and a closure delivery device are provided for closing incisions in a target tissue, such as an artery, vein, fascia or hollow organ.
