    • A system and method for opening a lumen in an occluded blood vessel, e.g., a coronary bypass graft, of a living being. The system comprises an atherectomy catheter having a working head, e.g., a rotary impacting impeller, and a debris extraction sub-system. The atherectomy catheter is located within a guide catheter. The working head is arranged to operate on, e.g., impact, the occlusive material in the occluded vessel to open a lumen therein, whereupon some debris may be produced. The debris extraction sub-system introduces an infusate liquid at a first flow rate adjacent the working head and withdraws that liquid and some blood at a second and higher flow rate, through the guide catheter to create a differential flow adjacent the working head, whereupon the debris is withdrawn in the infusate liquid and blood for collection outside the being's body. The introduction of the infusate liquid may also be used to establish an unbalanced flow adjacent the working head to enable the atherectomy catheter to be steered hydrodynamically. A guide wire having an inflatable balloon on its distal end may be used with the atherectomy catheter to block the flow of debris distally, while enabling distal tissues to be perfused with an oxygenating liquid. At least one flow control port may be provided in the guide catheter to prevent collapse of the vessel being revascularized. A cradle is provided to fix the guide catheter and guide wire in position within the body of the being while enabling the atherectomy catheter to be advanced along the guide wire and through the guide catheter.

    • A system and method for opening a lumen in an occluded blood vessel, e.g., a coronary bypass graft, of a living being.     • A tubular instrument, e.g., a catheter including a hollow wall portion forming the distal end of the instrument and terminating at a distal opening, having a connector arranged to be used with a guide-wire. The guide wire has a distal end portion and a proximal end portion, the distal end portion of the guide-wire being arranged to be extended to some interior position within the body of a living being, with the proximal end portion of the guide-wire being located outside the being's body. The connector is located at the distal end of the instrument and is arranged to be readily manipulated by a twisting action to connect the instrument onto the guide-wire for sliding movement therealong. In particular, the connector establishes a path into which a portion of the guide-wire can be inserted from a lateral direction by merely twisting the instrument, without requiring access to either end of the guide-wire. In one embodiment the path is a spiral channel formed in the hollow wall portion of the instrument. In another embodiment the connector is a spiral member located on the outside of the distal end of the instrument, spaced slightly from the outer surface of the instrument to form the path or channel into which the guide-wire is inserted.

The system furthermore features a shut-off valve on the extraction side so that certain injected fluids such as contrast medium, are not immediately pumped out of the patient. In a preferred embodiment, the system also features means for independently controlling the fluid pressure/volume on the infusion and extraction sides, self-priming capability, a continuous fluid path, and visual air bubble detection, with viewports located at important points in the fluid path, such as at pumps and valves.

    • Apparatus and methods of use for reducing bleeding from the situs of a percutaneous arterial puncture in a living being, with the opening in the arterial wall having been closed by a closure including at least one portion of a suture extending out of the puncture. The apparatus basically comprising a mass of material which inhibits the flow of blood therethrough, e.g., collagen, a carrier for carrying the suture portion through the mass of material, and means for holding the mass of material with respect to the suture portion so that it closely engages tissue contiguous with the puncture tract. The means for holding may comprise a knot in the suture portion, or can be a releasably securable member for location on the suture portion. In one embodiment the carrier comprises a piercing member, e.g., a needle, arranged for location within the interior of the artery for passage thereout through the wall of the artery and through the mass of material to carry the suture portion therethrough. In another embodiment the carrier comprises an elongated member, e.g., a flexible filament, arranged to selectively grasp the suture portion to carry it through the mass of material. A housing is provided for retaining the mass of material while the carrier extends the suture portion through it. A tamper may be provided for tamping the mass of material into intimate engagement with the contiguous tissue.

    • A system, a closure, and method of use for determining the position of a blood vessel via a percutaneous puncture and for sealing the percutaneous puncture in the blood vessel. The system includes an introducer sheath and associated positioning device, a hemostatic puncture closure, and a deployment instrument. The positioning device enables the introducer sheath to be positioned at a desired position within the vessel. The deployment instrument includes a tubular carrier storing the closure. The carrier has a distally located free end to be extended via an introducer through the puncture and its associated tract. The closure comprises a radiopaque rigid anchor, a compressed collagen plug, and a thin filament connecting the two in a pulley-like arrangement. The carrier ejects the anchor through the introducer and puncture and then draws it against the free end of the introducer. The instrument and introducer are then withdrawn together to pull the anchor against the tissue contiguous with the puncture inside the artery. Further withdrawl draws the plug out of the carrier into the puncture tract, whereupon the plug moves with respect to the anchor into engagement with the outside of artery wall to seal the puncture or incision. A tensioning device limits the force applied to the filament. The carrier also includes a tamper which is used to mechanically deform the plug within the tract. Once positioned hemostasis occurs rapidly, thereby locking the closure in place.

    • A system, a closure, and method of use for determining the position of a blood vessel via a percutaneous puncture and for sealing the percutaneous puncture in the blood vessel.
