    • An apparatus for detecting cardiac events in electrograms has a feature extraction unit that derives features of the cardiac events for discriminating among different types of cardiac events. A clustering unit groups cardiac events with similar features into respective clusters defined by predetermined cluster features. The feature extraction unit determines a feature vector describing waveform characteristics of cardiac events in the electrogram by a wavelet transform. The clustering unit determines the distance between the feature vector and corresponding cluster feature vectors in order to assign the cardiac event in question to that cluster which results in a minimum distance, providing that the minimum distance is less than a predetermined threshold value. A heart stimulator provided with such a cardiac event detecting apparatus detects the occurrence of an arrhythmia and appropriately controls a pulse stimulator to treat the arrhythmia.

    • In an implantable heart stimulating device and a method of the operation thereof, device has a control circuit that detects an evoked responses to delivered pacing pulses and to carry out an automatic capture routine. The control circuit is arranged to automatically temporarily disable the automatic capture routine on the basis of at least one of the following criteria: a1) if more than a predetermined number of threshold searches have been performed during a certain time, and b1) if a variable time delay with which the device operates is changed such that a pacing pulse may be delivered by the device during the evoked response time window.

    • In a method for identifying and classifying premature atrial contractions, a cardiac lead having multiple electrodes, electrically separated from each other, at its tip is placed in contact with cardiac tissue, so that at least two of the electrodes are simultaneously in contact with the cardiac tissue. When a depolarization wave arrives at the tip, respective unipolar signals are detected from multiple electrodes in contact with the cardiac tissue, and these signals are analyzed, such as by identifying a time offset between pairs of the signals, to identify a propagation direction of the depolarization wave. Dependent on this propagation direction, the depolarization wave is classified as representing a normal sinus beat or a premature atrial contraction. By further analysis, the premature atrial contraction, if present, is classified as originating either from the right atrium or the left atrium.
