    • A method and apparatus for monitoring glucose, ethyl alcohol and other blood constituents in a noninvasive manner. The measurements are made by monitoring infrared absorption of the desired blood constituent in the long infrared wavelength range where the blood constituent has a strong and distinguishable absorption spectrum. The long wavelength infrared energy emitted by the person as heat is monitored and the infrared absorption of particular constituents in the blood (such as glucose or blood alcohol) is measured at characteristic infrared absorption wavelengths for those constituents. The measurements are preferably synchronized with systole and diastole of the cardiac cycle so that the signal contribution caused by veins and tissues (which do not pulse) may be cancelled when a ratio of the detected signals is taken. On the other hand, if no synchronization is provided, the spectrophotometer may measure the arterial, venous and tissue constituent concentrations simultaneously, which may be desired in some circumstances. The internal "blackbody" energy level of an infrared emissions source such as a vascularized appendage prior to glucose absorption is measured and used to compensate temperature dependent effects in the concentration calculation. The internal energy level is then ratioed to the actual measured energy and used to compute the percentage of energy absorbed by the glucose. Discontinuities in the ratio are eliminated by converting the absorption measurements from voltages to watts.

    • A device and method are provided for use with a non-invasive optical measurement system, such as a thermal gradient spectrometer for improved determination of analyte concentrations within living tissue. In a preferred embodiment, a site selector is secured to a patient's forearm thereby isolating a measurement site on the patient's skin for determination of blood glucose levels. The site selector attaches to a thermal mass window of the spectrometer and thus forms an interface between the patient's skin and the thermal mass window. When the spectrometer must be temporarily removed from the patient's skin, such as to allow the patient mobility, the site selector is left secured to the forearm so as to maintain a consistent measurement site on the skin. When the spectrometer is later reattached to the patient, the site selector will again form an interface between the gradient spectrometer and the same location of skin as before.

To prevent the high energy source from burning or causing patient discomfort, only short bursts or pulses of energy are sent through the finger with a very low duty cycle and low optical bandwidth. The bursts are further synchronized with systole and diastole of the cardiac cycle so that only two pulses are sent per heart beat, one during diastole and one during systole. The detection signals measured during application of these bursts of energy are then used to calculate the concentration of the blood constituents in accordance with a polynomial equation.

    • A disposable endotracheal airway adapter for use in proximity of a patient's mouth in the measurement of the concentraton of respiratory gases. The disposable airway adapter of the invention provides a conduit for patient respiratory gases and allows the passage of infrared radiation through the gases for measuring the constituent concentration of the respiratory gases while keeping the patient gases from coming in contact with the analyzer mechanism. The body of the airway adapter is designed to connect in series with the airway tubing which connects a patient to a mechanical respirator or anesthesia breathing circuit. The body of the airway adapter is very light in weight and is formed of inexpensive material such as polyethylene or polypropylene so that it may be manufactured inexpensively and discarded after each use. An inexpensive optical window in the adapter is also formed of material such as polyethylene or polypropylene so that the gas analyzer can pass light into the sample for analysis. Preferably, the optical windows of the invention are made of the same material as the adapter body. Since the adapter of the invention may be disposed of after use on a single patient, sterilization expenses and cross-contamination may be minimized. Also, calibrating integrity is maintained through a calibration process which accounts for the non-linearity of the absorption spectra of the window material in the region of interest.

    • A disposable anti-fog airway adapter for use with a mainstream respiratory gas analyzer which provides a measurement of a patient's inhaled and exhaled gases. The airway adapter includes windows that are constructed of a thin, low heat capacity plastic that rapidly equilibrates to the temperature of the warm moist gases in the patient breathing circuit. In addition, the inside of the windows is also coated with an anti-fog surfactant either by laminating an anti-fog film with the window plastic prior to attaching the window to the airway adapter body or by first attaching the window to the airway adapter body and then applying the surfactant to the airway adapter after the window film is bonded in place so that the surfactant coats the entire inside of the adapter. The surfactant functions to increase the critical wetting tension of the surface it covers so that water on the window spreads into a uniform thin layer which does not absorb very much infrared energy and thus does not significantly reduce the signal strength. "Instant on" operation is accomplished because no heater and the like is necessary to warm up the windows to maintain them at an elevated temperature to prevent fogging. Numerous techniques are also provided for adhering the windows to the airway adapter body so that a substantially airtight seal may be obtained.
