    • The invention includes a series of surgical instruments (1000) providing at least one electrode (1002) for performing electrocautery, and provides a method for cutting and cauterizing tissue with a surgical instrument. The invention involves a method for detecting the location of a bleeding vessel in a liquid-filled, visually monitored surgical field of a patient and for electrocauterizing the vessel to stop the bleeding before visualization of the surgical field is compromised. Surgical instruments according to the invention also include operable components (118) for forming a liquid cutting jet for cutting or ablating tissue of a patient and/or for providing a rotating, tissue contacting component (122) for cutting, grinding, ablating, etc. tissue during a surgical procedure. Some surgical instruments include one or more liquid conducting lumen (118, 120) therein for transporting and/or removing a liquid from a surgical operating field, which lumen, in some cases, are selectively coated with a layer of an electrically insulating material so that certain, selected, uncoated regions of an external surface of the lumen can act as an electrocautery electrode of the instrument.

    • The present invention provides a series of devices for surgical procedures utilizing rotatable components for grinding, cutting, ablating, polishing, drilling, screwing, etc., tissues of the body of a patient. The invention includes, a series of surgical instruments (100) including rotatable shafts (124), and surgical components (122) drivable by the shafts (124) that can be utilized for contact with tissue in a surgical operating field. Some preferred surgical instruments provided by the invention utilize a liquid jet-driven rotor mechanism (454) for driving rotation of the rotatable shaft (124). Some preferred instruments provided by the invention include both a liquid jet-driven rotor mechanism (454) and a nozzle (192) at the distal end of the instrument for forming a liquid cutting jet for cutting or ablating tissue of a patient. Such instruments can include a liquid flow directing valve (180) therein that includes a pressure-tight sealing component (556) comprising a sealing element that is slidably moveable within a cylinder (552) of the valve (180). The invention provides methods for utilizing the surgical instruments in surgical procedures involving both cutting or ablating tissue of a patient with a liquid cutting jet and grinding, cutting, or ablating tissue with a rotating surface of a surgical instrument.

    • A device for treating obstructions in vessels or small openings in the body, comprising a rotatable ablator tip which is guided to the obstruction in a reduced diameter configuration, expanded and rotated to remove the obstruction, and contracted to remove the device from the body. The variably expandable abrasive tip coil in one embodiment is actuated by piston means within the coil. A pair of collars is attached to the ends of the coil, and the piston effects relative longitudinal axial movement of the collars and the respective ends of the coil tip. When the ends of the coil tip are so moved, expansion and contraction of the diameter of the coil tip results. In another embodiment, the expansion tip coil is actuated by an expandable and contractible bellows disposed within the coil. In another embodiment, the expansion and contraction of the coil tip are affected by longitudinal axial movement of an internal coil attached to one end of the coil tip, within an outer coil attached to its other end. In another embodiment, expansion and contraction of the coil tip are effected by an inflatable balloon disposed within the coil tip. The balloon expansion means enlarges preferably at the central portion of the coil to make a bulge. In alternative embodiments, there is provided a combined atherectomy and balloon angioplasty device employing either a fixed or variable diameter rotatable abrasive burr, and an angioplasty balloon disposed proximal to the burr.

    • A dental handpiece is disclosed including a turbine construction creating a radial in flow of air onto the impeller blades about the whole circumference of the turbine. This generates additional torque and avoids asymmetrical thrust on the impeller wheel. A pair of axially spaced air bearings support the turbine. Air supply to the bearings is controlled in such a way that the air bearings are floated before drive air is supplied to the turbine and after drive air to the turbine has been shut off. This ensures that the air bearings are always operational irrespective of the operational state of the turbine. The handpiece has an improved ergonomic shape, especially the shape and configuration of the front or drive head, which provides additional tooth clearance and a better field of view. An angled swivel connection to the umbilical cord is provided which reduces physical strain on the dentist's wrist. A self adjusting lock and key type torque connection between the dental burr and the chuck is provided which accommodates both conventional burrs and the burr of the lock and key arrangement. An auto stop arrangement for the turbine is disclosed which prevents a vacuum build­up during run-down of the turbine. The specific construction of the auto stop valve in accordance with the invention closes both the drive and exhaust air conduits.
