A screw member (42; 204; 404) has: a screw (44) extending into an aperture of the platform so that rotation of the screw about an axis (500) causes a shift of the platform and screw relative to the barrel parallel to the axis. One of the barrel and the screw member has a track (62; 124; 126; 126) circumscribing the axis and having a surface with an axial position varying with circumferential position about the axis. The other of the barrel and the screw member comprises a follower (124; 62; 144; 220) engaged to the track so that a continuous rotation of the screw member about the axis causes a reciprocal motion of the screw relative to the barrel. The track is shaped to provide two cycles of the reciprocal motion per each revolution of the screw member.
