摘要： An improved catheter and method for infusing blood to the heart, clamping the aorta and delivering a cardioplegia solution. A single catheter (10, 100) is used having a unique balloon (102, 162, 172) for occluding the aorta (12). In a first embodiment, a single catheter (10) is transthoracically inserted upwardly in the aorta to infuse oxygenated blood into the ascending aorta via an infusion opening (46) terminating at the distal end (32), with cardioplegia solution being delivered via openings (42) defined closely adjacent the balloon (40) on the proximal side of the balloon. In a second and third embodiment, a single catheter (100) is transthoracically and femorally inserted, respectively, inserted into the aorta to infuse blood via openings (102) into the ascending aorta proximate the Brachiocephalie Artery, and deliver cardioplegia solution via the distal opening (104) downward toward the aortic base. A fourth lumen (90, 140) is provided in both catheters for sensing aortic root pressure in combination with a pressure meter (92) to first ascertain if the balloon is sufficiently inflated and properly occluding the aorta, and second to ascertain proper function of the aortic valve during cardioplegia delivery. The balloon (202, 231) is preferably filled with a partitioned resilient material (204) to permit varying diameters of the balloon and occlude body passageways having varying diameters and curvatures, and may be elongated (262) to improve occlusion.

摘要： Disclosed is a device for isolating a cardiac surgical site. The device comprises a first finger (10) having a clinging accessory (30) for attaching the first finger to a heart (140), a second finger having a clinging accessory (30) for attaching the second finger (12) to the heart (140), a first joint disposed on the first finger (10) so that the first finger (10) may rotate on a surface of the heart such that said rotation stretches a surgical site, a first stopper (24) disposed on the first finger (10) for preventing undesired rotation of the first finger (10) to isolate the surgical site, and a link (18) for coupling the first finger (10) to the second finger (12). Several embodiments of the fingers and clinging accessories are disclosed. A guard (1602) is provided to protect sutures from the clinging accessory. The guard (1712) is equipped with a sprayer to wash the surgical site. Also disclosed is a method of isolating a cardiac surgical site. The method comprises the steps of disposing a first finger on a heart, clinging the first finger to the heart surface, disposing a second finger on a heart, clinging the second finger to the heart surface, and then rotating the first finger for achieving selective isolation of the heart surface.

摘要： A catheter and method of use adapted to intermittently deliver fluids to and drain fluids from a body vessel having an interior lumen occluding balloon. The selective inflation and deflation of the inner balloon controls the occlusion or communication of an inner lumen. The inner occluding balloon is defined between the distal end of the catheter and a proximal lumen opening allowing for the selective isolation of the proximal openings from the catheter distal end. The catheter of the present invention and several methods of use thereof are ideally suited for aortic perfusion and providing myocardial infusion in antegrade fashion using a single lumen. The catheter is further provided with a double lobed outer balloon for selectively occluding the body vessel, such as the aorta, and to provide a bloodless region to complete the proximal end of an anastomosis while perfusing the myocardium to restart the heart.
