    • A clot retrieval device (1) has a clot engaging portion configured at the distal end of an elongate shaft (6) having an outer expandable member (2) comprising multiple expandable segments (26), and an inner expandable member (3) to facilitate restoration of blood flow through clot immediately after the clot retrieval device (1) is deployed at an obstructive site.     • A clot retrieval system comprises a clot retrieval device, a catheter 51, and a tubular shield element 61. The clot retrieval device comprises an elongate member 53 having a proximal end and a distal end, and a clot engaging element 54 at the distal end of the elongate member 53 the clot engaging element 54 having a collapsed delivery configuration and an expanded deployed configuration. The catheter is adapted for placement at a location at which a distal end of the catheter is proximal of the clot engaging element and the tubular shield element 61 extends over the elongate member 53 and is adapted for location between the distal end of the catheter and the proximal end of the clot engaging element 54. The tubular shield element 61 is movable relative to the elongate member 54 when the clot engaging element is in the expanded configuration in that the force that is applied on retraction of the elongate member is spread over the outer surface of the tubular shield element. The force that would usually be exerted on vessels on retrieval of the clot engaging element is spread by the shield element thus reducing the effects of abrasion.

    • A catheter 101 provides a proximal seal 102 against a guide catheter inner lumen 103 so that aspiration may be applied through a guide catheter 108 and thus take advantage of the large proximal lumen. The catheter 101 is a rapid exchange (RX) catheter with an exit port 105 that defines a transfer port for aspiration and provides a deliverability advantage of minimal frictional engagement with the guide catheter 108 proximal of the exit port 105. Aspiration can be applied to the lumen of the guide catheter 108 and is directed to and effective at the tip of the Rx aspiration catheter 101 due to the presence of flow a restrictor or seal 102 between the outer surface of the Rx aspiration catheter 101 and the inner guide catheter 108. This seals the lumen between the outside of the Rx aspiration catheter 101 and the inner surface of the guide catheter 03 and prevents backflow of blood into the tip of guide catheter 108 reducing the effectiveness of the aspiration. An expansile tip 112 of the Rx catheter 101 facilitates aspiration and retrieval of the clot 110 by expanding under load. The expansile tip 112 can also partially or fully occlude the vessel providing flow arrest and thereby improving aspiration effectiveness.

    • A clot removal device (2000) for removing clot from a body vessel comprises an expandable structure and an elongate member. The expandable structure has a constrained delivery configuration, an expanded clot engaging deployed configuration, and an at least partially constrained clot pinching configuration. At least a portion of the expandable structure is configured to engage clot in the expanded deployed configuration and to pinch clot on movement from the deployed configuration to the clot capture configuration. The expandable structure may comprise a main body portion (2002) and a clot pinching structure (2001) and wherein a diameter of the clot pinching structure (2001) is less than a diameter of the main body portion (2002). The clot pinching structure in some cases is substantially tubular and may be of spiral form.
