摘要： Systems and methods provide for pacing a heart to improve pumping efficiency of the heart, such as by producing a cardiac fusion response for patient's subject to cardiac ^synchronization therapy. A pacing parameter, such as an A-V delay, V-V delay, lead/electrode configuration or vector, is adjusted and a cardiac signal vector representative of all or a portion of one or more cardiac activation sequences is monitored during pacing parameter adjustment. A change in a characteristic of the cardiac signal vector is detected in response to an adjusted pacing parameter, the change indicative of a cardiac fusion response. A pacing therapy may be delivered to produce the cardiac fusion response using the adjusted pacing parameter.

摘要： Respiration-synchronized heart sound trends can be detected using an implantable medical device, including a respiration sensor, a respiration phase detector, a heart sound sensor, a heart sound detector, and a processor. The implantable medical device can also include a cardiac sensor. The respiration-synchronized heart sound trends can include heart sounds occurring during specific phases of a respiration signal, such as inspiration or expiration. The heart sound signal can be gated or the heart sound sensor can be enabled or disabled using the cardiac sensor or the respiration sensor. Further, the heart sound trends can be displayed using an external display, and can provide information about a cardiovascular status using an analysis module, or the analysis module and a blood volume sensor.

摘要： A CRM system enhances intracardiac electrogram-based arrhythmia detection using a wireless electrocardiogram (ECG), which is a signal sensed with implantable electrodes and approximating a surface ECG. In one embodiment, an intracardiac electrogram allows for detection of an arrhythmia, and the wireless ECG allows for classification of the detected arrhythmia by locating its origin. In another embodiment, the wireless ECG is sensed as a substitute signal for the intracardiac electrogram when the sensing of the intracardiac electrogram becomes unreliable. In another embodiment, a cardiac signal needed for a particular purpose is selected from one or more intracardiac electrograms and one or more wireless ECGs based on a desirable signal quality. In another embodiment, intracardiac electrogram-based arrhythmia detection and wireless ECG-based arrhythmia detection confirm with each other before indicating a detection of arrhythmia of a certain type.

If the first composite value is outside of a first specified range, then a second composite value is calculated based on at least a second sensor measurement, wherein the second composite value is an indication of a likelihood that the patient's heart failure status has changed. If the second composite value is outside of a second specified range, then an alert of change in heart failure status is generated, wherein the alert includes an indication of a likelihood that the patient's heart failure status has worsened. The first and second specified ranges can be modified.

The analyzer is configured to detect laryngeal vibrations or cough from the sensed impedance values.
