    • An endoscope has two or more branches and each branch may include a source of illumination and a lens train, fiber optic bundle, or solid state image receiving device. Each branch end is independently manipulable or steerable and thus produces an image from a distinct point of view within the body (e.g., front and side views); the images are juxtaposed on a video monitor for simultaneous viewing by the surgeon. Each branch also includes an operating channel or cannula through which various surgical instruments are passed. A first branch may be used to pass an instrument to the situs of a surgical procedure and a second branch may be used to view that situs from a second, separate point of view. Additionally, the second branch may be used to introduce a second surgical instrument through the second branch cannula to approach the situs of the procedure from the angle of the second branch.

    • An instrument for suturing anatomical tissue with a suture needle includes a housing, an elongate shaft having a proximal end mounted by the housing and a distal end with a peripheral edge, a first needle holder protruding from the distal end of the elongate shaft and having a needle holding member at a distal end operable to grasp and release a suture needle, and a second needle holder protruding from the distal end of the elongate shaft and having a needle holding member at a distal end operable to grasp and release a suture needle. Distal portions of the first and second needle holders extend laterally outward at an angle from first and second longitudinal axes of the elongate shaft to positions where at least a portion of the corresponding needle holding members are spaced laterally outward of the peripheral edge of the elongate shaft. In addition, the needle holders are rotatable about the first and second longitudinal axes of the elongate shaft to cause the corresponding needle holding members to move along first and second arcuate paths. When inserting the suturing instrument through a portal in an endoscopic procedure, the first and second needle holders are preferably movable to undeployed positions where their needle holding members are spaced laterally inward of the peripheral edge of the elongate shaft. When suturing, the needle holding members can be moved from their undeployed positions to deployed positions disposed laterally outward of the peripheral edge due to the angled configuration of the distal portions of the needle holders.

    • An endoscopic portal for establishing communication with a body cavity for the introduction of instruments through the endoscopic portal includes a seal having a variable size passage for communication with the body cavity to permit instruments of varying sizes to be passed therethrough and a tubular expander aligned with the variable size passage. The seal is biased to a contracted position wherein the variable size passage has a first cross sectional size and is movable to an expanded position wherein the variable size passage has a second cross sectional size larger than the first cross sectional size. The tubular expander has a blunt end insertable in the variable size passage to move the seal from the contracted position to the expanded position such that an instrument having a cross sectional size larger than the first cross sectional size can be introduced through the expander and the seal into the body cavity without contacting the seal. Thereafter, the blunt end of the tubular expander is withdrawable from the variable size passage such that the seal engages the instrument in the variable size passage to form a seal therewith. A method of introducing an instrument in a body cavity according to the present invention comprises the steps of moving a blunt end of a tubular expander into a variable size seal to enlarge the variable size seal, introducing an instrument through the tubular expander into the body cavity such that a distal end of the instrument is moved through the seal without contacting the seal and withdrawing the blunt end of the tubular expander from the seal such that the seal sealingly engages the instrument.

    • An expandable endoscopic portal for providing a passage through a body cavity wall includes a cannula comprising an absorbent member having a distal end for positioning in the body cavity, a proximal end for positioning external of the body cavity and a passage between the distal and proximal ends for receiving instruments. The absorbent member has a compressed dry state prior to introduction through the cavity wall and an expanded wet state when supplied with fluid upon introduction of the distal end in the body cavity. The absorbent member is rigid in the dry state to facilitate passage through the cavity wall and is soft in the wet state. The absorbent member has an initial cross sectional size in the dry state for introduction through an opening in the cavity wall corresponding in size to the initial cross sectional size. The absorbent member in the wet state has a cross sectional size greater than the initial cross sectional size to dilate the opening and/or form a seal along the thickness of the cavity wall. Instruments of various cross sectional sizes can be introduced in the body cavity through the passage in sealing relation. A method of establishing a passage through a cavity wall includes introducing an elongate absorbent member in an opening in the cavity wall with the absorbent member in a dry state such that the absorbent member extends longitudinally through the cavity wall and hydrating the absorbent member to place the absorbent member in a wet state to cause the absorbent member to expand radially within the opening.

The suture spring device includes an elastic body which is of coiled configuration in the contracted state to define a single ring, a portion of a ring or a plurality of connected rings having a radius of curvature and/or axial spacing in the relaxed, contracted state which is smaller than the predetermined radius of curvature and/or axial spacing of the guide so that, when the suture spring device is disposed within the guide, it is maintained in an elastically deformed, expanded state for positioning in anatomical tissue.
