    • An air introduction system for a medical needle assembly is provided and includes a flexible bladder, and a male luer assembly with an air valve which is biased to allow air to enter the air introduction system but not to exit. The air introduction system is coupled to a needle assembly which is to be introduced into the epidural space. As the needle traverses the tissue layers overlying the epidural space, and the practitioner applies pressure to the bladder, the practitioner experiences resistance to the forcing of air through the needle assembly. If air is lost from the bladder, the bladder may be refilled by releasing the pressure on it and allowing outside air to enter via the air valve. Once the epidural space is penetrated, air is quickly forced from the bladder through the needle assembly and the practitioner can feel a sudden drop in pressure. To aid in detection, the air introduction system is provided with an air activated sound device situated so as to be exposed to the air flow pushed out of the bladder. With the sound device, an audible warning is provided as the air rushes from the bladder through the needle assembly and into the epidural space.

    • An endoscopic tool for dispensing units of hemostatic agent broadly comprises a hollow tube which holds a plurality of individual units of the hemostatic agent, a valve at the distal end of the hollow tube which permits the hemostatic agent to pass therethrough but prevents foreign matter from contacting the hemostatic agent while in the tube, a plunger which extends into the hollow tube and contacts a proximal unit of the hemostatic agent, and a mechanism for moving the plunger distally in incremental movements to cause, upon each incremental movement, an individual unit of the hemostatic agent to be pushed through the valve. Typically, the mechanism for moving the plunger and the proximal end of the hollow tube are held in a handle. One mechanism for moving the plunger utilizes a portion of the plunger as a ratchet by forming a plurality of conical elements thereon, and utilizes a switch in the handle which has a V-spring coupled thereto as a pawl for advancing the conical elements. A second mechanism utilizes a regularly shaped notched cutout in the handle as the ratchet, and a vertical protrusion on the plunger which extends through the cutout as an engagement element. The notched cutout provides resilient stops, and the protrusion is shaped to allow it to be forced in a forward direction through each resilient stop and then to sit in another notch.

    • A trocar catheter includes a trocar rod having a stepped diameter distal end for use with a catheter having a stepped diameter lumen wherein the trocar is inserted into the lumen of the catheter and the trocar is used to incise a patient and install the catheter after which the trocar is removed from the lumen of the catheter. The distal end of the trocar rod is provided with a stepped coaxial bore into which a piercing point rod, a spring and a safety sleeve are assembled. The piercing point rod has a proximal small diameter portion which is press fit into a correspondingly small diameter portion of the coaxial bore in the distal end of the trocar rod. The spring is carried on the small diameter portion of the piercing point rod and biases the safety sleeve in a distal direction. The distal end of the piercing point rod has a larger diameter portion tapering to a piercing tip and the safety sleeve has a stepped inner diameter which prevents it from moving beyond a predetermined distal position covering the piercing tip. The stepped coaxial bore has a large diameter portion for receiving the safety sleeve when it is pressed proximally against the spring. A protrusion on the distal outer surface of the safety sleeve prevents the catheter from accidentally slipping off the trocar.

    • A coaxial bone marrow biopsy needle assembly is disclosed. The assembly includes a hollow cannula with a handle having an interlocking device and a trocar insertable in the cannula and having a second handle with another interlocking device which mates with the interlocking device in the cannula handle. The interlocking devices are a spring biased pin radially entering a receiving socket, and a notched projection which extends into the receiving socket. The pin has a ramped portion, and the notched projection has a rounded or conical tip which rides down the ramped portion of the pin and thereby pushes the biased pin radially out of the socket during assembly of the hollow cannula with the trocar. The assembled handles provides a smooth upper surface which fits comfortably in a palm and a lower gripping surface around which fingers easily wrap. The interlocking devices hold the handles securely together keeping the trocar within the cannula during puncture, but are easily separated thereafter.

    • The needle assembly of the present invention includes a hollow cannula having a sharp distal end, a hollow stylet having a blunt distal end with a radial opening and an interior deflection surface near the radial opening, and a mechanism for biasing the stylet in a first position where the blunt distal end of the stylet extends beyond the sharp distal end of the cannula. Preferably, the hollow cannula hub has a proximal end which is coupled, e.g., via insert molding, to a hollow cannula hub. Likewise, the stylet preferably includes a proximal end which is coupled, e.g., via insert molding, to a hollow stylet hub which is longitudinally movable within the hollow cannula hub. The mechanism for biasing typically comprises a spring inside the cannula hub which biases the stylet hub to the first position where the blunt distal end of the stylet extends beyond the sharp distal end of the cannula. The stylet is movable against the spring to a second position where the sharp distal end of the cannula extends beyond the blunt distal end of the stylet. A first indicator is provided on the cannula hub showing the position of the stylet relative to the cannula and a cannular orientation indictor on the cannula shows the direction of the radial opening.

    • A bone marrow needle assembly includes a cannula housing supporting a cannula having a sharp distal end. The cannula housing is provided with a central throughbore in fluid communication with the cannula and with an ergonomic handle having a curved distal surface. A female luer lock fluid coupling is provided in the housing at the proximal end of the throughbore and within the periphery of the curved handle. A trocar is located within the cannula and extends through the luer lock and the throughbore. The trocar has a cap which extends over the luer lock and is rounded so that a substantially continuous rounded surface is presented by the semi-circular handle and the trocar cap. The distal end of the housing is provided with a threaded portion which receives a locking nut and a depth stop. The depth stop has a mating interior threaded bore which couples with the threaded distal portion of the housing so that the depth stop may be rotated to adjust the effective piercing length of the cannula. The locking nut is rotated into a locking position against the depth stop to secure the depth stop in a selected position.

    • A probe for use with an endoscopic suction-irrigation instrument is disclosed which permits a second probe to be inserted therethrough. The suction-irrigation instrument has a cannula, a fluid chamber having at least one port for irrigation and suction and a proximal opening, and a first seal in the proximal end of the fluid chamber. The fluid chamber has a distal end communicating with the cannula. The probe has a proximal handle portion, a middle portion, and a hollow distal portion. The handle portion has an axial passageway and a second seal therein. The middle portion extends from the handle portion through the first seal at the end of the fluid chamber and through the fluid chamber. The middle portion has an outer diameter shaped to contact the first seal such that the proximal end of the fluid chamber is sealed. The middle portion is also shaped to be smaller than the inner diameter of the fluid chamber, and the middle portion is provided with a second axial internal passageway extending therethrough. The distal portion extends at least partially through the cannula and has an outer diameter smaller than the inner diameter of the cannula. A second probe may be inserted through the second seal of the first probe, through the axial passageway in the handle portion, through the internal passageway in the middle portion, and out through the distal portion and the cannula.
