    • A bipolar electrosurgical scalpel comprising a handle having a pair of loop electrodes extending outwardly from the distal end thereof in parallel, spaced relation, such that the spacing defines a dielectric such as an air gap therebetween. When a RF voltage of a predetermined amplitude is applied across the two electrodes and they are drawn across tissue, cutting occurs by virtue of the arc established between the two electrodes. The electrodes may be formed from tungsten wire to withstand high operating temperatures and are sufficiently rigid to withstand the pressure forces encountered during electrosurgery without shorting together across the gap. The electrodes may be formed in various shapes, depending upon the nature of the cutting desired. Possible configurations include open rounded loops, open elongated loops, open triangular-shaped loops, L- or J-shaped hooks. The supporting conductors to which these electrodes are affixed may be straight tubular legs or curved hooks formed from flattened lengths of metal tubing. The loop electrodes may also be ultrasonically vibrated and/or fluid flushed to enhance removal of char and tissue debris during use. To enhance coagulation properties, metal surface electrodes of relatively large area may be embedded in an insulating end cap of the scalpel and the loop cutting electrodes are designed to be retracted therein such that only the surface electrodes are exposed. The instrument is controlled by either a hand switch or a standard surgical foot switch.

    • An electrosurgical implement for tunneling through tissue for effecting drainage of an organ or to restore patency to a blood vessel is described. The implement comprises a stainless steel, helically wound, wire body similar to a conventional guidewire of the type used in angiography, angioplasty or in implanting cardiac pacing leads. The exterior of the guidewire is coated with an insulating material, save for one or two convolutions at its distal end. A core wire having a Teflon.RTM. shrink tube surrounding it over substantially its entire length is disposed within the lumen of the helical guidewire and only a short segment of the distal end of the core wire is allowed to extend out from the distal end of the guidewire. This short segment is stripped free of insulation. The implement may be routed through an endoscope into the stomach or colon and when an RF voltage of an appropriate amplitude is connected across terminals electrically joined to the proximal ends of both of the guidewire and the core wire, an arc discharge is created when the distal end of the implement is brought into contact with tissue, thereby allowing the device to readily pierce this tissue. The same implement may also be used for restoring patency to a blocked blood vessel.

    • A bipolar electrosurgical cutting and coagulating instrument for cutting and coagulating tissue in open surgery. The instrument comprises a handle member from which an electrically conductive first forceps rod and an electrically conductive second forceps rod extend in opposing relationship to each other. An electrically conductive wire extends from the handle member to reside within a longitudinal lumen of the second forceps rod. The first forceps rod is partially movable proximally within the handle member wherein a movable electrical conduit resides. Proximal movement of the first forceps rod causes the rod to contact and move the conduit which, in turn, causes distal movement of the conductive wire with which the conduit is in contact. Such distal wire movement causes the distal end of the wire to extend distally from the second forceps rod. When the wire is extended, the distal end thereof functions as a scalpel since bipolarity is established between the distal end of the wire and the second forceps rod. When the wire is retracted, a coagulation mode results since bipolarity is established between the first and second forceps rods.

    • A surgical instrument for facilitating the injection of a contrast fluid into the cystic duct during the course of a laparoscopic cholecystectomy procedure comprises an elongated, rigid outer tubular member whose outer diameter permits it to pass through the lumen of a trocar penetrating the patient's abdominal wall and of a length allowing the distal end of the instrument to abut the patient's cystic duct. Affixed to the proximal end of the outer tubular shaft is a grip that includes a plunger which can be reciprocally moved in the longitudinal direction and joined to the plunger is an inner tube which can be joined at its proximal end to a source of contrast fluid and which extends through the lumen of the outer tubular shaft. Affixed to the distal end of the inner tube is a hollow hook-shaped needle which is in fluid communication with the lumen of the inner tube and which has a sharpened distal end for piercing through the wall of the cystic duct when the plunger is depressed to thereby hook and hold the instrument with the open end of the needle in the cystic duct. By injecting contrast fluid into the proximal end of the instrument, it flows through the inner tubular member and the hollow needle into the cystic duct.
