    • This invention relates to tracheostomy tubes and particularly to a tracheostomy tube construction and a system incorporating a talking tracheostomy tube. A pneumatic vibrator is provided for generating sonic vibrations. A tracheostomy tube includes a main airway and secondary airway. The secondary airway extends generally along the main airway for a substantial portion of its length. Furthermore there is an entryway to the secondary airway outside the entrance of the tracheostomy tube into a tracheostomy. And an exit from the secondary airway adjacent its end remote from the entryway. A source of compressed air is coupled to the pneumatic vibrator and the pneumatic vibrator coupled to the entryway to supply air containing sonic vibrations to permit the wearer of the tracheostomy tube to articulate audibly comprehensible speech.

    • A pneumatic speech simulator has a vibratory tone-generating element which produces an audible tone in response to the flow of pressurized air within a predetermined pressure range, said tone being transmitted to a patient's pharyngeal region by a conduit, such as a nasal catheter. The simulator includes a pressure compensation valve, upstream from the tone-generating element, which maintains the pressure experienced by the tone-generating element within its operating range as the pneumatic resistance of tone-transmitting conduit is increased due to partial or total blockage.

    • A talking tracheostomy tube according to the teachings of the present invention is shown in the preferred embodiment as including an inner cannula removably received within an outer cannula. The tracheostomy tube further includes, in the preferred embodiment, a first cuff for sealing between the outer cannula and the trachea for substantially preventing air introduced through the inner cannula to escape from the trachea through the larynx. In the preferred embodiment, air is introduced into the trachea above the first cuff by ports directionally drilled through the outer cannula and in fluid communication with a secondary passageway formed by and between the inner and outer cannulas. The inner end of the secondary passageway is formed by a sealing obstruction integrally formed in the inside surface of the outer cannula adjacent the inner end for the sealing receipt of the inner cannula. The outer end of the secondary passageway is sealed by a collar integrally formed on the outer end of the inner cannula which abuts and seals within a recess of a collar integrally formed on the outer end of the outer cannula. In its preferred form of the present invention, the tracheostomy tube further includes a second cuff for sealing between the outer cannula and the stoma of the patient for preventing the secondary air introduced through the ports from escaping through the stoma in the neck of the patient.

    • An apparatus and device for permitting a person with a tracheostomy to speak while being ventilated. A tracheostomy tube having an orifice in its tubular wall and a valve means in operative association with the orifice are provided. A valve controlling means is arranged to cause the valve means to block the orifice when a ventilator which is connected to the tracheostomy tube feeds inhalation oxygen thereto, and to open the orifice when exhalation occurs, thereby allowing exhaled breath to flow upwardly in the trachea past the larynx to permit speech. The valve means may comprise a balloon mounted inside of a hood.
