    • An instrument for performing a surgical procedure includes a housing, an elongate shaft having a proximal end mounted by the housing and a distal end with a peripheral edge, a first end effector assembly protruding from the distal end of the elongate shaft and having an end effector at a distal end, and a second end effector assembly protruding from the distal end of the elongate shaft and having an end effector at a distal end. Distal portions of the first and second end effector assemblies extend laterally outward at an angle from first and second longitudinal axes of the elongate shaft to positions where at least a portion of the corresponding end effectors are spaced laterally outward of the peripheral edge of the elongate shaft. In addition, the end effector assemblies are rotatable about the first and second longitudinal axes of the elongate shaft to cause the corresponding end effectors to move along first and second arcuate paths. When inserting the surgical instrument through a portal in an endoscopic procedure, the first and second end effector assemblies are preferably moved to undeployed positions where their end effectors are spaced laterally inward of the peripheral edge of the elongate shaft, for example by rotating the end effector assemblies inwardly or drawing the end effectors proximally into the elongate shaft so that they are straightened. In use, the end effectors can be moved from their undeployed positions to deployed positions disposed laterally outward of the peripheral edge due to the angled configuration of the distal portions of the end effector assemblies.

    • An endoscopic portal for establishing communication with a body cavity for the introduction of instruments through the endoscopic portal includes a seal having a variable size passage for communication with the body cavity to permit instruments of varying sizes to be passed therethrough and a tubular expander aligned with the variable size passage. The seal is biased to a contracted position wherein the variable size passage has a first cross sectional size and is movable to an expanded position wherein the variable size passage has a second cross sectional size larger than the first cross sectional size. The tubular expander has a blunt end insertable in the variable size passage to move the seal from the contracted position to the expanded position such that an instrument having a cross sectional size larger than the first cross sectional size can be introduced through the expander and the seal into the body cavity without contacting the seal. Thereafter, the blunt end of the tubular expander is withdrawable from the variable size passage such that the seal engages the instrument in the variable size passage to form a seal therewith. A method of introducing an instrument in a body cavity according to the present invention comprises the steps of moving a blunt end of a tubular expander into a variable size seal to enlarge the variable size seal, introducing an instrument through the tubular expander into the body cavity such that a distal end of the instrument is moved through the seal without contacting the seal and withdrawing the blunt end of the tubular expander from the seal such that the seal sealingly engages the instrument.

The forceps engage the clip in the grasping position and are movable from a closed position to an open position to move the clip from the grasping position to the receiving position to receive the structure between the grasping surfaces. The forceps are movable from the open position to the closed position to permit the clip to move from the receiving position toward the grasping position to grip the structure between the grasping surfaces. A cartridge storing a plurality of multifunctional spring clips is insertable in the applicator for selectively releasing one of the clips from the cartridge into engagement with the forceps.
