摘要： A steerable elongated medical device that is capable of forming as “s” shaped bend and having a guidewire tip at its distal end. The devices include an outer tube extending between an outer tube proximal segment and an outer tube distal segment, having an outer tube wall forming an outer tube lumen and first and second elongated outer tube slots through the outer tube wall to the outer tube lumen. The first elongated outer tube slot has a first portion and a second portion and formed between a first outer tube slot proximal end and a first outer tube slot distal end and extending axially along the outer tube distal segment through a first outer tube slot length to define a first cutaway portion of the outer tube. The second elongated outer tube slot has a first portion and a second portion and formed between a second outer tube slot proximal end and a second outer tube slot distal end and extending axially along the outer tube distal segment through a second outer tube slot length to define a second cutaway portion of the outer tube and offset about 180 degrees from the first elongated outer tube slot. Each of the first and second outer tube slots have reinforcing sleeves, each of the reinforcing sleeves having reinforcing sleeve slot portions and reinforcing sleeve overlap portions. The medical devices further include a handle coupled to the outer tube proximal end, and a pull wire positioned within the outer tube lumen and extending between a pull wire proximal end coupled to the handle and a pull wire distal end comprising a guidewire tip, the pull wire extending through the first and second reinforcing sleeve lumens, the outer tube lumen, and a distal pull wire stop, and the guidewire tip extending distally from the distal pull wire stop, wherein the pull wire proximal end is adapted to be manipulated to separate the pull wire proximal end from the outer tube proximal end to induce a bend in each of the first and second cutaway portions.

摘要： Methods and systems for transvenously accessing the pericardial space via the vascular system and atrial wall, particularly through the superior vena cava and right atrial wall, to deliver treatment in the pericardial space are disclosed. A steerable instrument is advanced transvenously into the right atrium of the heart, and a distal segment is deflected into the right atrial appendage. A fixation catheter is advanced employing the steerable instrument to affix a distal fixation mechanism to the atrial wall. A distal segment of an elongated medical device, e.g., a therapeutic catheter or an electrical medical lead, is advanced through the fixation catheter lumen, through the atrial wall, and into the pericardial space. The steerable guide catheter is removed, and the elongated medical device is coupled to an implantable medical device subcutaneously implanted in the thoracic region. The fixation catheter may be left in place.

摘要： An improved system and method for placing implantable medical devices (IMDs) such as leads within the coronary sinus and branch veins is disclosed. In one embodiment, a slittable delivery sheath and a method of using the sheath are provided. The sheath includes a slittable hub, and a substantially straight body defining an inner lumen. The body comprises a shaft section and a distal section that is distal to, and softer than, the shaft section. A slittable braid extends adjacent to at least a portion of one of the shaft section and the distal section. In one embodiment of the invention, the sheath further includes a transition section that is distal to the shaft section, and proximal to the distal section. The transition section is softer than the shaft section, but stiffer than the distal section.

摘要： A guide catheter has properties that promote enhanced visibility of the guide catheter using fluoroscopic or ultrasonic imaging techniques. The tip of the guide catheter may incorporate a material that is both fluoro and echo visible. The first material also may be provided in various amounts along the length of the guide catheter. In addition, the guide catheter may incorporate a reinforcing braid that includes one or more strands of a second material that is radio-opaque. As a result, the result guide catheter achieves improved radio-opacity and echogenicity.

摘要： Curves and dynamic shapes are imparted in distal segments of medical instruments e.g., catheters, through use of a temperature-activated actuator coupled to distal segment of the instrument body. The actuator comprises includes members extending side-by-side in substantially a common plane and configured to move in opposite directions when independently activated. A first heating device is coupled to a first actuator member and configured to deliver thermal energy to the first actuator member to activate the first actuator member and cause the first actuator member to move the distal segment in a first direction. A second heating device is coupled to a second actuator member and configured to deliver thermal energy to the second actuator member to activate the second actuator member and cause the second actuator member to move the distal segment in a second direction opposed to the first direction.

An engagement cavity receives and properly orientates the lead distal tip to align the lumen of the lead with the opening of the navigation portion during the loading of the guide wire within the lead. A first side wall is spaced from a second side wall to form a slot extending along the outer portion from the front end to the back end of the navigation portion, and the guide wire is advanced through the slot as the lead having the guide wire loaded therein is removed from the loading device.
